Crosstalk Between Mucosal Inflammation and Bone Metabolism in Chronic Rhinosinusitis

Abstract: Chronic rhinosinusitis (CRS) is a multifactorial and highly heterogeneous upper airway disease that affects approximately 12% of the general population. There is increasing evidence supporting the impact of osteitis on the pathophysiology of CRS. Osteitis is frequently observed in patients with CRS, and is associated with severe sinonasal inflammation and recalcitrant cases. The overlying inflammatory sinonasal mucosa plays a critical role in the initiation of osteitis; however, the underlying molecular mechan… Show more

“…Generally, NECRSwNP is regarded as an extrinsic form of CRS, since its inflammation originates from external stimuli such as bacteria and allergens, rather than intrinsic mucosal abnormalities [ 27 ]. Thus, NECRSwNP usually shows enhanced epithelial alterations and more localized maxillary involvement than ECRSwNP [ 28 , 29 ].…”

“…Collectively, the increased heterogeneity in immune status may be associated with more severe disease expression, remodeling, and poor response to corticosteroids in patients with ECRSwNP and moderate-to-severe OSA. Generally, NECRSwNP is regarded as an extrinsic form of CRS, since its inflammation originates from external stimuli such as bacteria and allergens, rather than intrinsic mucosal abnormalities [27]. Thus, NECRSwNP usually shows enhanced epithelial alterations and more localized maxillary involvement than ECRSwNP [28,29].…”

“…More than one million different endophytic fungal strains have been reported to inhabit about 300,000 plant species [6]. Most fungal endophytes that have been reported belong to the Basidiomycota or Ascomycota [7,8]. Fungal endophytes function as stimulants of plant growth by synthesising phytohormones, enhancing nutrient accessibility by mobilising soil insoluble nutrients, and by protecting host plants against abiotic and biotic stress [9,10].…”

“…PBMP contains both phosphomonoester groups for calcium binding and butyl groups for hydrophobic PLGA binding, respectively, and can be coated on the hydrophobic surface of PLGA microparticles to introduce an adhesive property on calcium-rich tissues. We selected dexamethasone (Dex) as a model corticoid drug, which can reduce the inflammatory sinonasal mucosa [ 19 ] and promote wound repair following the surgical procedure [ 20 ]. We intended to maintain the Dex concentration for a longer duration only near the exposed bones after the surgery through sustained Dex release from the PLGA/PBMP particles, which have a specific binding affinity to the calcium-rich tissues ( Figure 1 ).…”
