Background. According to International Association for the Study of Pain (IASP), the prevalence of neck-shoulder pain in general population is the fourth leading cause of disability. Neck-shoulder pain is known to be caused by different conditions. The aim of this study was to determine and statistically justify the correlation between shoulder girdle injuries and development of neck-shoulder pain syndrome. Methods. The study enrolled 1402 patients with hospital-treated shoulder girdle injuries (with consequences of upper limb trauma). Detailed protocol and questionnaire for clinical examination after discharge from the hospital were conducted. The prevalence of neck-shoulder pain among patients was calculated. The diagnostic value (sensitivity and specificity) of tests used in the study was assessed. Results. The prevalence of neck-shoulder pain was 66,9%. According to questionnaire results (VAS, ODI, SF-36, UCLA, SPADI, DASH), the tendency of increasing pain intensity, progressive upper extremity functional loss and, therefore, quality of life impairment in patients with shoulder girdle injury was noticed. Conclusion. 3 types of neck-shoulder pain syndrome in patients with shoulder girdle injury can be determined: articular, vertebrogenic and mixed (myofascial), which correlate with anatomical localization of the injury.

Musculoskeletal mimics of cervical radiculopathy will be explored in this AANEM monograph. A review of the literature and textbooks was conducted. Musculoskeletal cervical radiculopathy mimics include: (1) head/neck pain, such as neck tension, cervicogenic headache, and temporomandibular disorder; (2) referred pain from shoulder pathology, such as rotator cuff tears/impingement; (3) elbow region abnormalities, including medial/lateral epicondylitis; (4) wrist/hand conditions, such as DeQuervain 0 s tenosynovitis and intersection syndrome; (5) muscle connective tissue disorders, including myofascial trigger points; (6) conditions that have decreased range of motion, including frozen shoulder and rounded shoulders with tight pectoral/scalene muscles;(7) conditions with joint hyperlaxity and instability, as seen in post-stroke shoulder subluxation; (8) vascular conditions, such as thoracic outlet syndrome; and (9) autonomic controlled soft tissue changes associated with complex regional pain syndrome.Formulating a differential diagnosis and recognizing mimics of cervical radiculopathy can decrease the time between symptom onset and diagnosis/treatment plan.

Shoulder pain is a common musculoskeletal disorder that has a significant impact on the patient’s quality of life and functional health. Because the shoulder joint is a complex structure, the relevant symptoms of shoulder pain may not directly reflect the underlying pathology. Hence, several shoulder tests have been developed to examine shoulder problems, and their diagnostic accuracy varies. Familiarization with the performance of those physical tests and their strength and limitation are of utmost importance for physicians dealing with shoulder disorders. Therefore, the present narrative review focuses on summarizing the most commonly used tests in physical examination and their diagnostic performance on several shoulder pathologies. This article also discusses how ultrasound imaging can serve as an extension of those tests.
