Background:The aim of the present study was to determine the therapeutic effect of phentolamine combined with Ambroxol aerosol inhalation on pediatric severe pneumonia and its effect on serum interleukin-10 (IL-10) and C-reactive protein (CRP) levels.Methods: Eighty-five children with severe pneumonia treated in our hospital from November 2019 to November 2020 were selected as the research participants, and were divided into the routine group (n=41) and treatment group (n=44) according to odd and even admission numbers, respectively. Children in the first group received routine treatment, namely symptomatic treatment such as cough relief (e.g., expectorant) and defervescence, while those in the second group received phentolamine combined with Ambroxol aerosol inhalation. Clinical indexes of both groups before and after treatment were analyzed to determine the therapeutic effect of different treatment methods and serum IL-10 and CRP level changes.Results: There was no significant difference in general clinical data between the 2 groups (P>0.05). The duration of cough, fever, abnormal lung sound and lung shadow, and hospitalization time in the treatment group was significantly shorter than those in the routine group (P<0.001). The total clinical effective rate in the treatment group was significantly higher than that in the routine group (P<0.05). Forced vital capacity and peak expiratory flow rate levels were higher in both groups after treatment (P<0.05), and these were higher in the treatment group compared with the routine group after treatment (P<0.05). Serum IL-10 and CRP levels at T1 (2 days after treatment), T2 (5 days after treatment), and T3 (7 days after treatment) in the treatment group were significantly lower than those in the routine group (P<0.001). The total incidence of adverse reactions in the treatment group was significantly lower than that in the routine group (P<0.05).Conclusions: Phentolamine combined with Ambroxol aerosol inhalation can significantly improve the clinical symptoms of children with severe pneumonia, reduce the body's inflammatory response, and improve lung function safely.

Favipiravir and Meropenem have been concurrently used as directly acting antiviral and antibiotic agents for the treatment of coronavirus disease in human plasma. Accurate and specific reversed phase ultra-performance liquid chromatographic and high-performance thin-layer chromatographic methods were developed and validated for the first time analysis of this combination in spiked human plasma using Cefepime as an internal standard. In the developed ultrahigh-performance liquid chromatography method, separation was performed on a BEH C18 column with a mixture of ACN and 0.05 M potassium dihydrogen orthophosphate (pH = 3) in a ratio of 10:90 (v/v) as an eluate. Scanning of the separated peaks was at 298 nm. The developed method showed high sensitivity, and the drugs showed linearity in the range of 5-70 μg/ml for Favipiravir and 2-50 μg/ml for Meropenem. The proposed high-performance thin-layer chromatographic method included the separation using a mixture of ethyl acetate:methanol:deionized water:formic acid (5:4:1.5:0.3, by volume), then spots detection at 300 nm. Methods were investigated for greenness using the eco-scale and national environmental method index tools and were validated according to food and drug administration guidelines. Methods can be applied for bio-analysis and therapeutic drug monitoring studies.

Introduction: A retrospective study aimed at analyzing the patterns in the CT scans of patients of CKD infected with COVID-19 in the period of two months. We reviewed the CT scans for the pattern of Lung involvement, number of lobe involved, distribution of the lesions and characteristic appearances, along with other coexisting features. Methodology: After consent, we studied the parameters of interest among the patients admitted in the dedicated COVID-19 hospital in Bengaluru, Karnataka, who were admitted during the months of October and November 2020. Each CT scan was reported by in-house radiologist and results were analyzed. Results: We found that of the 92 CKD patients, 51 had got a CT thorax done, out of which 45 had lung parenchymal involvement of COVID-19; of which 44 were bilateral. 26 of the patients had only peripheral lesions and 19 had both central and peripheral distribution. The most common finding was GGOs occurring in 43 of them while 30 of them had GGOs with consolidation. This study shows that the CT patterns in patients of CKD with COVID-19 involves mainly peripheral distribution of lesions with majority involving >2 lobes, with GGOs being most common finding; similar to those without CKD, but the incidence of pleural effusions and mediastinal lymphadenopathy is slightly higher, possibly because of coexistent comorbidities contributing to the same. Conclusion: We conclude that though some patients with CKD and COVID-19 can have a normal lung, the involvement of lungs is more severe and extensive in distribution, implying a more severe involvement of the lung. Hence, further Studies are required on larger population to ascertain the findings and further resources should be allocated for the benefit of this significantly vulnerable patients in this era.
