Background. In Germany—as worldwide—headache is one of the most frequent causes of self-medication. The dispensing of over-the-counter (OTC) medications may only be carried out by community pharmacies (CPs). In doing so, CPs have to ensure “adequate” counseling, for both self-purchase and purchase for a third party, which also occurs in everyday pharmacy practice. The aim of this study was to evaluate the quality of counseling for headache OTC medications in German CPs and, as the first study worldwide, to analyze whether and to what extent there are differences in counseling between self-purchase and purchase for a third party. Methods. A cross-sectional study was carried out using the covert simulated patient methodology (SPM) in all 42 CPs in the German big city Potsdam. With the help of 8 trained simulated patients (SPs), each CP was visited four times by a different SP. The SPs simulated in each CP two scenarios two times with the demand for an OTC medication against headache, which differed only in whether the demand was for themselves or for their boyfriend/girlfriend. Results. All 168 planned pharmacy visits (84 visits per scenario) were successfully carried out. Overall, the median counseling score was 3.0 out of 9 points (interquartile range [IQR 2.0]). There were no significant differences between the two scenarios (Wilcoxon signed-rank test; p = 0.495 , r = 0.053). In a multivariate binary logistic regression analysis, the counseling level and the different scenarios were not significantly associated (adjusted odds ratio [AOR] = 1.635, 95% CI = 0.673–3.972, p = 0.278 ). Conclusions. Due to the partly considerable deficits in counseling of German CPs, policy-makers and the regional chambers of pharmacists are called upon to take appropriate measures to improve the quality of counseling. It is positive that no differences in counseling between self-purchase and purchase for a third party were found, but further verifying studies with a modified methodology are recommended.

Evaluating Patient Safety Culture in Iraqi Community Pharmacies

Patient safety is the main issue in health care organization, the Agency for Healthcare Research and Quality defines it as, “freedom from accidental or preventable injuries produced by medical care. Thus, practices or interventions that improve patient safety are those that reduce the occurrence of preventable adverse events”. The purpose of this study was to evaluate Iraqi pharmacist perception about the culture of patient safety. As well as estimate whether safety is a principal issue in their pharmaceutical practice this study was carried out on 435 pharmacists who are working in community pharmacies in various Iraqi provinces. A survey was distributed via the internet during the period from May to June 2020. A community pharmacy questionnaire was used to evaluate the awareness of pharmacists regarding the culture of patient safety. A result of this study shows that the patient counseling field was the most positive one among the studied domains with score 68.8% of positive awareness and 70.4% of the pharmacists indicated that they inform patients with needed information about their new prescriptions. In contrast, staffing and work pressure scored the lowest positive response (36.55%). Although 66.7% of the participants stated they have the appropriate number of staff in their pharmacies to deal with the workload.

Dược sĩ cộng đồng (DSCĐ) là nhân tố quan trọng trong vận hành hệ thống y tế đảm bảo sức khỏe cộng đồng nên cần đảm bảo kiến thức chuyên môn và trau dồi kỹ năng thực hành nghề nghiệp (KNTHNN). Tổng quan hệ thống các kết quả đánh giá kỹ năng thực hành nghề nhiệp của nhân viên bán hàng tại các nhà thuốc tư nhân trên thế giới là cần thiết nhằm cung cấp bức tranh toàn cảnh về đặc điểm, kỹ năng hành nghề cũng như các yếu tố liên quan đến KNTHNN của dược sĩ cộng đồng. Phân tích tổng quan hệ thống được thực hiện theo tiêu chuẩn PRISMA trên cơ sở dữ liệu Pubmed, Cochrane và Research Gate bằng câu lệnh, từ khóa và tiêu chí lựa chọn, loại trừ phù hợp. Dữ liệu về đặc điểm các nghiên cứu và đánh giá KNTHNN của DSCĐ được trích xuất và tổng hợp. Trong 6 nghiên cứu được chọn, có 4 nghiên cứu sử dụng phương pháp mô phỏng đóng vai người mua, 2 nghiên cứu khảo sát tự đánh giá và quan sát bí mật. Kỹ năng hỏi thu thập thông tin từ người bệnh có tỉ lệ chênh lệch cao 1,7% - 73,3% giữa các quốc gia. Kỹ năng tư vấn - cung cấp thông tin của DSCĐ ở Việt Nam có kết quả thấp nhất với chỉ 10% và không một DSCĐ nào khuyên khách hàng đến chuyên gia sức khỏe, trong khi đó kỹ năng này ở các quốc gia châu Âu và Úc là khá cao. Kỹ năng tư vấn của DSCĐ rất cao ở Đức và yếu kém ở Việt Nam và Jordan. Kết quả này giúp định hướng những nhận định phù hợp hơn trong công tác giáo dục đào tạo cũng như quản lý dược sĩ cộng đồng tại Việt Nam.
