Costs of autologous and allogeneic hematopoietic cell transplantation in the United States: a study using a large National Private Claims Database

Abstract: There is a lack of multi-center cost-identification studies for hematopoietic cell transplantation (HCT). We used a single longitudinal administrative claims database representing a national, commercially insured population to evaluate the feasibility of identifying HCT recipients and to establish a cohort of autologous and allogeneic HCT recipients to study inpatient and outpatient direct medical costs from transplant hospitalization through first 100 days post-transplantation. Using ICD-9 procedure and diagn… Show more

“…5,7,[9][10][11] A more recent comparative study of autologous and allogeneic HSCT for patients transplanted for hematologic malignancies in the USA estimated a 100-day total cost of US$ 203,026 for allogeneic HSCT. 12 Unrelated donor cord blood transplantation (UCBT) has become a widely accepted transplant modality in the absence of an HLA-matched donor. [13][14][15][16] However, the delay of engraftment and the increased risk of graft failure remain problems in adults transplanted with a single cord blood unit.…”

“…92,99,100 Among pediatric patients, one study found that older children (above age ten) had higher costs, but overall, pediatric patients also tend to be more costly. 100,101 Costs also varied by the condition for which the patient was being treated, but all these factors are likely to affect decisions about conditioning, graft type, and HSC source, as well as health care setting. Moreover, HSC transplants in inpatient settings will typically be reimbursed based on the episode, which may include a risk adjustment for patientlevel factors.…”
