Costello Syndrome with Severe Nodulocystic Acne: Unexpected Significant Improvement of Acanthosis Nigricans after Oral Isotretinoin Treatment

Abstract: We report the case of 17-year-old female diagnosed with Costello syndrome. Genetic testing provided a proof with G12S mutation in the HRAS gene since 3 years of age with a presentation of severe nodulocystic acne on her face. After 2 months of oral isotretinoin treatment, improvement in her acne was observed. “…Another report was of improvement with sustained maintenance of AN lesions on combination therapy of isotretinoin and metformin 66 . A patient of Costello syndrome showed resolution of AN lesions on neck and hand following treatment by isotretinoin for nodulocystic acne 28 . A case of paraneoplastic AN with gastric adenocarcinoma also showed improvement of lesions after treatment with retinoids 67…”

“…Established modalities of treatment of acanthosis nigricans have been topical use of retinoids, α-hydroxy acids, or salicylic acid in adjunct with treatment of underlying condition. [1] Among the orally used drugs, insulin sensitizers such as metformin, rosiglitazone,[16] sitagliptin,[17] and oral retinoids such as isotretinoin and acitretin[18] have been assessed to have some effect in reduction of cutaneous lesions in acanthosis nigricans. However, to the best of our knowledge, effectiveness of alpha-lipoic acid in acanthosis nigricans has not yet been studied.…”
