Cost Savings of Standardization of Thoracic Surgical Instruments: The Process of Lean

Abstract: Standardization of thoracic surgical trays is possible despite having multiple thoracic surgeons. This process of lean (the removal of nonvalue steps or equipment) reduces the number of instruments cleaned and carried and reduces cost. It may also reduce the incidence of "wet loads" that require the resterilization of instruments.

“…For example, most surgeons at our institution perform laparoscopic appendectomies similarly and use the same items. Studies have shown that standardization of preference cards can lead to decreased costs across multiple procedures and specialties [20][21][22] , so perhaps cost utilization in such procedures are already maximized. Another consideration in the analysis of our data is the effect of time.…”

“…More importantly, stakeholders must sustain these changes after the initial enthusiasm has waned. In previous studies [17,18], lean and value stream mapping was successful in providing high-quality, efficient patient care for high-risk procedures. The lessons learned from these previous projects were instrumental in designing this new study for a New York City health care system and culture.…”
