ORIGINAL ARTICLE PURPOSE The aim of this study was to evaluate the 10-gauge vacuum-assisted stereotactic biopsy (VASB) of isolated Breast Imaging Reporting and Data System (BI-RADS) 4 microcalcifications, using histology and follow-up results. METHODSFrom January 2011 to June 2013, VASB was performed on 132 lesions, and 66 microcalcification-only lesions of BI-RADS 4 were included into our study. VASB was performed using lateral decubitis stereotaxy for all patients. Pathologic results of VASB and further surgical biopsies were reviewed retrospectively. Patients who were diagnosed to have benign lesions by VASB were referred for follow-up. VASB and surgical histopathology results were compared to determine the underestimation ratios. RESULTSFifteen out of 66 lesions from 63 patients (median age, 47 years; range, 34-88 years) were identified as malignant by VASB. Pathological results after surgery revealed three cases of invasive ductal carcinoma among the 12 VASB-diagnosed ductal carcinoma in situ (DCIS) lesions, for a DCIS underestimation rate of 25%. The atypical ductal hyperplasia underestimation rate was 0% for the three lesions. The follow-up period was at least 10 months, with an average of 22.7 months for all patients and 21.2 months for patients with VASB-diagnosed benign lesions. None of the patients had malignancy during the follow-ups. The false-negative rate was 0% in the follow-up of 48 patients. CONCLUSION VASB should be the standard method of choice for BI-RADS 4 microcalcifications. This method obviates the need for a surgical procedure in 73% of BI-RADS 4 microcalcification-only patients.

SUMMARYRecently, the advanced breast biopsy instrumentation (ABBI) system has been introduced as an alternative to conventional breast biopsy techniques. This study was prospectively conducted to evaluate the potential of the ABBI method in locoregional management of a consecutive series of patients with nonpalpable mammographically detected breast carcinomas. Sixty-one consecutive patients underwent an ABBI procedure as a first step before possible surgery for nonpalpable breast lesions that would in any case require complete excision. For the 27 patients in whom the ABBI biopsy revealed malignancy further surgery was recommended, including re-excision of the biopsy site and axillary dissection in cases of infiltrating carcinoma. We calculated the probabilities that the ABBI specimen would have tumor-free margins and that a definitely complete excision had been achieved as a function of the mammographic or pathological diameter of the cancer. For cancer with a pathological diameter less than 10 mm measured on the ABBI specimen, the probability (92%) of obtaining complete resection was significantly better than for larger lesions (P = 0.01, Fisher's exact test). Although the therapeutic perspectives for the ABBI method are limited at present, we suggest that this approach is a first step in the direction of a surgical strategy that is better adapted to the pathological characteristics peculiar to these small tumors, whose incidence is increasing.

Bildgesteuerte minimal-invasive Nadelbiopsien stellen in der Diagnostik von Mammakarzinomen ein etabliertes Verfahren dar, welches bei korrekter Indikation und qualitätsgesicherter Durchführung der diagnostischen offenen Operation ebenbürtig, wenn nicht sogar überlegen ist. Eine offene diagnostische Biopsie sollte daher nur im begründeten Ausnahmefall durchgeführt werden. In Abhängigkeit von der Sichtbarkeit des histologisch abzuklärenden Befundes in den unterschiedlichen diagnostischen Verfahren muss die Nadelbiopsie mit dem bildgebenden Verfahren durchgeführt werden, in dem die suspekte Mammaläsion am sichersten identifiziert werden kann. Ist eine Läsion sowohl mammographisch wie auch sonographisch gleichermassen nachweisbar, so ist unter Berücksichtigung der Belastung der Patientinnen und der dabei entstehenden Kosten der sonographisch gesteuerten Hochgeschwindigkeitsstanzbiopsie der Vorzug zu geben. Bei suspekten Mikrokalzifikationen muss die Nadelbiopsie unter stereotaktischer Kontrolle erfolgen. Hier liefern die vakuumunterstützten Verfahren [Mammotome^®, VacuFlash (BIP)/Vacora (Bard)] im Vergleich zur Hochgeschwindigkeitsstanzbiopsie deutlich bessere Ergebnisse und sollten in diesen Fällen, wie auch bei nur in der Magnetresonanztomographie sichtbaren Läsionen, bevorzugt Anwendung finden. Nicht palpable und in der bildgebenden Diagnostik suspekte, oder besser schon präoperativ durch minimal-invasive Verfahren histologisch als maligne gesicherte Läsionen sollten ausnahmslos präoperativ mit Draht markiert werden. Nur dadurch kann sichergestellt werden, dass die suspekte und nicht palpable Läsion intraoperativ sicher gefunden wird und mit ausreichendem Sicherheitssaum bei möglichster Schonung des gesunden Gewebes operativ entfernt werden kann. Die qualitätsgesicherte Diagnostik und Therapie des Mammakarzinoms sind eine interdisziplinäre Herausforderung und liefern optimale Ergebnisse nur bei optimaler Zusammenarbeit aller daran beteiligten Disziplinen.
