Corticosteroid-Free Immunosuppression with Tacrolimus, Mycophenolate Mofetil, and Daclizumab Induction in Renal Transplantation

Abstract: Corticosteroid-free immunosuppression with a Dac/Tac/MMF regimen is as effective at preventing acute rejection after renal transplantation as a standard triple regimen of Tac/MMF/corticosteroids. Furthermore, the safety benefits reported with Dac/Tac/MMF treatment may help improve the long-term outcome for renal-transplant patients.

“…The de novo use of hypoglycemic agents was less in the steroid-free group (4.5%) compared to the steroid withdrawal (12.2%) or standard steroid group (14.7%). The CARMEN study randomized 538 kidney transplant recipients to daclizumab/tacrolimus/MMF with steroid maintenance compared to the same regimen without steroids (18). The incidence of NODAT was 5.9% in patients maintained on steroids compared to 0.4% in the steroid-free regimen (p ¼ 0.003).…”

“…Whereas successes with early steroid withdrawal have been limited by the risk of acute rejection and the perceived or actual reduction of longterm outcomes, some recent trials have been published with encouraging results with early graft survival and acute rejection incidence similar to steroid-based protocols, especially when concomitant antibody induction is administered (1), dosages of cyclosporine are high (2,3), or maintenance therapy consists of tacrolimus and/ or mycophenolate mofetil (4,5).…”

“…The feasibility of complete avoidance of steroids in adults after renal transplantation was first suggested by an open label Canadian multicenter trial [5]. Since then, reports from other centers have also confirmed the efficacy and safety of various steroid minimization or avoidance regimens in adults [6][7][8][9]. These studies have also provided evidence that renal allograft recipients managed with less steroid exposure experience benefits such as a lower incidence of hypertension, diabetes, weight gain and hyperlipidemia.…”
