New-Onset Diabetes After Transplantation:Results From a Double-Blind Early Corticosteroid Withdrawal Trial

Abstract: New-onset diabetes after transplantation (NODAT) is an important complication following kidney transplantation. Data from the 5-year early steroid withdrawal double-blind randomized trial were analyzed to determine if steroid avoidance reduced the NODAT risk. Incidence, timing and risk factors for NODAT were evaluated using eight definitions. By American Diabetes Association definition, 36.3% of patients on chronic corticosteroids (CCS) and 35.9% on early corticosteroid withdrawal (CSWD) were diagnosed with NO… Show more

“…Randomized trials of early corticosteroid withdrawal in adult renal transplant recipients showed limited impact on NODAT [12, 13], whereas lower rates of hyperglycemia and NODAT were reported in pediatric renal transplant recipients following early corticosteroid withdrawal [14, 15]. The current study showed no significant association between monthly cumulative steroid exposure and NODAT.…”

“…High-dose corticosteroids, often used as part of induction protocols in the immediate post-transplant hospitalization, have much greater impact than chronic low-dose corticosteroids that are common to many maintenance immunosuppression protocols. A recent prospective randomized trial of early withdrawal of corticosteroids vs remaining on low-dose chronic prednisone (5 mg/d) from 6 months to 5 years after kidney transplant showed that incidence of PTDM was minimally impacted (36).…”
