Correlations of placental vascular anatomy and clinical outcomes in 69 monochorionic twin pregnancies

Abstract: Monochorionic monozygotic twins frequently suffer complications from the presence of vascular anastomoses in their monochorionic placentas. Also, sharing of perfusion zones may be unequal, leading to marked growth discordance. This paper analyzes four measures of perinatal outcome (gestational age at delivery, perinatal mortality, birth weight discordance, and presence/absence of hydramnios) according to the vascular patterns of the monochorionic placentas. The worst clinical outcomes were associated with arte… Show more

“…Approximately a third of MZ twins are DCMZ, and two thirds, MCMZ twins (Bulmer, 1970). The sharing of a chorion and a placenta, and the presence of vascular anastomoses between the circulations of the two fetuses allows exchange of blood, hormones, oxygen and other material between both members of the twin pair (Machin et al, 1996). For this reason, MCMZ twins can be more similar than DCMZ twins in postnatal development.…”

“…The anastomoses may vary in number and type, that is, arterio-arterial, veno-venous or arterio-venous or a combination of these, and the variation affects the clinical outcome (Machin et al, 1996). The flow patterns across these anastomoses may be either balanced or imbalanced with adverse clinical outcomes predominantly being with the latter.…”

“…La presencia de Aa-v, no compensada por Aa-a protectora (7), ante diferencia de presión intraplacentaria, origina transfusión unidireccional, de mal pronóstico perinatal y su expresión máxima es el Síndrome de Transfusión Gemelo-Gemelo (STGG) grave, presente en 15% de los casos y que tiene evolución letal (8,9).…”
