Correlation of the Kennedy Osteitis Score to clinico‐histologic features of chronic rhinosinusitis

Abstract: The KOS is a simple, easy, and reproducible scale in assessing osteitic bones in patients with CRS and can predict measures of severity in eosinophilic rhinosinusitis.

“…401,402 A variety of grading systems using CT imaging have also been proposed, including the Kennedy Osteitis Score, Global Osteitis Scoring Scale (GOSS), scoring based upon Hounsfield units (HU), thickness of bony partitions, as well as systematic examinations of separate reference points in the sinuses. 387,389,392,403,404 As with diagnostic testing for osteitis, these grading systems have not been standardized.…”

“…Nevertheless, the mechanisms that trigger osteitic bone changes are still unclear. Infection, inflammation, biofilm, and increased number of surgeries are among the implicated factors . Cytokines, such as tumor necrosis factor‐α, interleukin (IL)‐1, IL‐6, and IL‐11 have been considered to have roles in the inflammatory process .…”

“…Identification of patients with elevated P-gp may therefore be useful to guide treatments directed toward this novel therapeutic target. Radiographic osteitis scores 2,3 have been previously shown to correlate with eosinophilic inflammation and thus may be useful in stratifying patients with P-gp overexpression. P-gp is a 170-kDa glycoprotein encoded by the MDR1 (ABCB1) gene located on chromosome 7q21.12.…”

“…Osteitis involves inflammatory changes in the underlying bone that lead to persistence of disease and is increasingly recognized as playing a significant role in recalcitrant CRS. 6 Snidvongs et al 3 showed that both the Kennedy Osteitis Score (KOS) and the Global Osteitis Score (GOS) were capable of predicting disease severity in eosinophilic inflammation. The purpose of this article was to determine whether osteitis scores may be used as a noninvasive clinical marker of P-gp overexpression in patients with CRS.…”
