Dengue, endemic in India presents in mild to severe forms with or without bleeding. There is no specific treatment available till date hence severe dengue has to be recognized in early stages. Platelet counts are unreliable as they do not correlate with bleeding. Platelet indices have been studied for their utility as predictors of severity of the disease and aid in guide to transfusion therapy. Mean platelet volume is inexpensive and available with routine complete blood counts in hematology analyser. Its role as marker in diagnosis, prognosis and treatment of dengue has been investigated with platelet indices reports. To study MPV patterns, association with platelet counts and relevant laboratory parameters in dengue. The study was conducted over one month in the year 2016 in department of Hematology, Kempegowda Institute of Medical Sciences Hospital, Bengaluru. 100 serologically proven dengue cases were analysed with data collected from the records in the departments of Hematology and Microbiology against patient’s unique hospital identification number. There was a predominance of young male patients. The range for MPV was 8.7 – 13.2 (average 10.95) with 35% normal and 65% high MPV cases. There was 48% cases with moderately severe / severe thrombocytopenia. 32% with increased hematocrit, 35% with leucopenia, 54% with significant atypical lymphocytosis and 42% with antibody pattern. There was an increased number of high MPV seen in severe thrombocytopenia, increased hematocrit significant atypical lymphocytosis and antibody pattern whereas normal pattern was noted in leucopenia. High MPV is a useful predictor of severe dengue platelet recovery and may guide in transfusion therapy.

Background: Dengue is a major international public health concern. Various clinical and biochemical parameters are being used to predict severity of this disease. Blood platelets and their indices are one of them. Platelet indices like mean platelet volume (MPV), platelet distribution width (PDW), and platelet large cell ratio (P-LCR) have been investigated as prospective platelet activation indicators. Aims and objectives: To find out the relationship between the platelet indices at admission and morbidity profile of dengue patients. Materials and methods:The study was an observational study conducted on patients of pediatric age group in Department of Pediatrics, Dr SN Medical College, Jodhpur, over a period of 1 year. All ELISA (IgM + IgG) positive cases were enrolled in the study. Results: In this study high MPV, high PDW, and high P-LCR were associated with longer duration of hospital stay and longer duration of fluid therapy requirement. These patients required ICU care more frequently, more time to recover from severe thrombocytopenia (TCP), and had deranged SGPT and serum creatinine values, which were commonly seen with high MPV. Shock was more frequently seen with high MPV, and bleeding manifestations were more common with low MPV. Almost 80% patients were admitted with low plateletcrit (PCT), which was associated with longer duration of hospital stay, longer duration of fluid therapy, required ICU care more frequently, required more time to recover from severe TCP, landed up in shock more frequently, and required more blood product transfusion. High MPV, high P-LCR, low PCT, and severe TCP were associated with more frequent positive tourniquet test with a highly significant p value. Bleeding manifestations and neurological symptoms were seen more with low MPV. Conclusion:We observed significant correlation of P-LCR, PDW, and MPV on admission with those morbidity parameters. High MPV, high P-LCR, high PDW, and low PCT were associated with longer duration of hospital stay, fluid therapy requirement, time taken to recover from TCP, frequent requirement of ICU care, and more frequent liver and renal function dysfunctions. They had higher chances of developing dengue shock syndrome.

Background: The study analyzed clinical characteristics and platelet indices in 200 children (1 month to 15 years) diagnosed with dengue fever, admitted to affiliated hospitals. It aimed to explore the relationship between platelet indices and disease severity in pediatric patients of teaching hospitals affiliated with MRMC College with a confirmed diagnosis of dengue fever. Methods: In this study, the majority of patients were aged (10-15) years, with a small percentage (3.5%) being infants. Females accounted for 50% of the cases. Fever was the most common symptom, followed by headache, abdominal pain, and myalgia. Rash occurred in (14.5%) of cases. Among 200 cases, 80 exhibited warning signs, with varying positivity for NS1 and IgM. All patients required fluid therapy, and (77.5%) needed ICU care. Platelet count and Plateletcrit decreased initially but improved later, while PDW, MPV, and PLCR increased and then decreased. NS1-positive cases had lower platelet count and Plateletcrit but higher PLCR and MPV compared to IgM-positive cases. Results: The study found that lower platelet count and Plateletcrit, as well as higher mean platelet volume, were associated with more severe disease outcomes, including longer hospital stays, increased intensive care unit care, and higher fluid therapy requirements. However, there was no significant correlation between other platelet indices (PLCR and PDW) and disease severity indicators. Conclusions: In conclusion, this study provided valuable insights into the clinical features and platelet indices of patients with dengue fever. Understanding the relationship between these indices and disease severity can help in predicting the progression of the illness and improving patient management strategies.
