Correlation Between Upper Limb Volume and Arterial and Venous Blood Flow Velocity in Lymphedema Secondary to Breast Cancer Treatment

Most tissue‐engineered blood vessels are endothelialized by static cultures in vitro. However, it has not been clear whether endothelial cell‐shedding and local damage may occur in an endothelial layer formed by static cultures under the effect of blood flow shear postimplantation. In this study, we report a bionic and cost‐effective vascular chip platform, and proved that a static culture of endothelialized tissue‐engineered blood vessels had the problem of a large number of endothelial cells falling off under the condition imitating the human arterial blood flow, and we addressed this challenge by regulating the flow field in a vascular chip. Electrospun membranes made of highly oriented or randomly distributed poly(ε‐caprolactone) fibers were used as the vascular scaffolds, on which endothelial cells proliferated well and eventually formed dense intima layers. We noted that the monolayers gradually adapted to the artery‐like microenvironment through the regulation of chip flow field, which also revealed improved cellular orientations. In conclusion, we have proposed a vascular chip with adaptive flow patterns to gradually accommodate the statically cultured vascular endothelia to the shear environment of arterial flow field and enhanced the orientation of the endothelial cells. This strategy may find numerous potential applications such as screening of vascular engineering biomaterials and maturation parameters, studying of vascular biology and pathology, and construction of vessel‐on‐a‐chip models for drug analysis, among others.

The Study on Dorsal Venous Arch of Upper Limb Among Nepalese Medical Students at Tertiary Level Teaching Institute

Introduction: Dorsal venous arch of upper limb is the beginning site for cephalic and basilic veins. The superficial veins are clinically important for venipuncture, blood collection and blood donation as well as for health screening and testing, intravenous transfusion for the emergency treatment of hypovolemic shock. Veins of the upper limb are also used for total parental nutrition, therapeutic invasive procedure and blood samples. The Dorsal Venous Arch is superficially placed and can be easily made prominent by putting a tourniquet at the wrist for any such surgical and therapeutic purposes. Methods: It was a cross-sectional observational study conducted in 200 hands among 100 individuals from MBBS first and second year students studying at a Medical College. Body Mass Index (BMI) of individuals was calculated to observe the correlation between length of the dorsal venous arch and BMI. Results: The mean±s.d. length of dorsal venous arch in male on right hand (20.7±2.4 cm) was significantly greater than in female on right hand (19.4±2.1 cm).There was no significant difference in length of dorsal venous arch on left side between male (20.1±2.2 cm.) and female (19.3±1.8 cm). Conclusions: The length of the dorsal venous arch on right hand was found significantly greater in male than in female. Also the length of dorsal venous arch on right side was found to be more than on left side in both sexes. The male students are significantly older, heavier and taller than females.

The Use of Ultrasound Imaging for Upper Extremity Lymphedema after Breast Cancer: A Systematic Review

Background The aim of this study was to analyze the different applications of ultrasound (US) in upper extremity lymphedema (UEL) after breast cancer. Methods A systematic review of the literature was performed in line with the PRISMA statement using MEDLINE/PubMed databases from January 1970 to December 2021. Articles describing the application of US in patients with UEL after breast cancer were included. The quality of the study, the level of reproducibility, and the different applications and type of US technique were analyzed. Results In total, 30 articles with 1,193 patients were included in the final review. Five different applications were found: (1) diagnosis of UEL (14 studies found a direct correlation between lymphedema and morphological and/or functional parameters); (2) staging/severity of UEL (9 studies found a direct correlation between the clinical stage and the soft-tissue stiffness/texture/thickness); (3) therapeutic assessment (3 studies found an improvement in the circulatory status or in the muscle/subcutaneous thickness after conservative treatments); (4) prognosis assessment of UEL (1 study found a correlation between the venous flow and the risk of UEL); and (5) surgical planning (3 studies determined the location of the lymphatic vessel for lymphovenous anastomosis [LVA] surgery). Conclusion Morphological and functional parameters have been correlated with the diagnosis, stage, therapeutic effect, prognosis of UEL, and surgical planning of LVA.
