Menopause is the consequence of the exhaustion of ovarian follicles which results in decreased production of estradiol and other hormones. Menopause is associated with a significant increase in cardiovascular diseases (CVD). Postmenopausal women have higher blood sugar values as compared to the control group because the estrogen hormone which makes the body cells more receptive or sensitive to insulin is either not secreted at all or is in limited supply. The estrogen hormone in females is protective for developing diabetes. Estrogen seems to contribute to glucose homeostasis in women. Significant negative correlation between fasting blood sugar and HDL with (r=-0.130, p= 0.369), suggesting that the fasting blood glucose levels were elevated whereas HDL levels were low normal in postmenopausal women thus associate with high risk of CVD in postmenopausal women.

Objective: Cardio-and cerebro-vascular diseases are two leading causes of death and long-term disability in postmenopausal women. The acute fall of estrogen in menopause is associated with increased cardiovascular risk. The relative contribution of androgen to this risk is also being recognized. The use of more sensitive assays for estradiol measurement and the study of receptor and carrier protein gene polymorphisms have provided some new information on the clinical relevance of endogenous sex steroids. We provide an update on the role of endogenous sex steroids on cardio-and cerebro-vascular disease in the postmenopausal period. Design and methods: We performed a PubMed search using the terms 'endogenous estrogen', 'androgen', 'cardiovascular disease', 'cerebro-vascular disease', 'stroke', 'carotid artery disease', and 'subclinical atherosclerosis'. Results: The majority of studies show a beneficial effect of endogenous estrogen on the vasculature; however, there are a few studies reporting the contrary. A significant body of literature has reported associations of endogenous estrogen and androgen with early markers of atherosclerosis and metabolic parameters. Data on the relevance of endogenous sex steroids in heart disease and stroke are inconclusive. Conclusions: Most studies support a beneficial role of endogenous estrogens and, probably, an adverse effect of androgens in the vasculature in postmenopausal women. However, the described associations may not always be considered as causal. It is possible that circulating estrogen might represent a marker of general health status or alternatively reflect the sum of endogenous androgens aromatized in the periphery. Elucidating the role of sex steroids in cardio-and cerebro-vascular disease remains an interesting field of future research.

Background: Menopausal health in our environment has received little attention. As a independent risk factor for dyslipidemia, the degree and pattern of derangement, though difficult to assess may adversely affect the cardiovascular health of our women.Objectives: To estimate the serum lipid profile and the atherogenic index of plasma among the pre and post- menopausal women.Materials and Methods: After an overnight fasting blood samples were collected from a group of 339 women, 140 premenopausal aged between 25-50 years and 199 postmenopausal aged between 51-70 years. Serum total cholesterol (TC), triglycerides (TG) and HDL-cholesterol were estimated by enzymatic methods and LDL-cholesterol by established mathematical methods. Atherogenic index of plasma (AIP) were calculated by using the formula (logTG/HDL-C). Statistical analysis was carried out in the two groups using the unpaired t test. Results were expressed as mean±SD. P values <0.05 were considered to be statistically significant.Results: There were statistically significant increase in serum TC (191.21±45.50 mg/dl), TG (185.83± 111.83 mg/dl) and LDL-C (118.71±38.48 mg/dl) in post-menopausal women. Their HDL-C level (38.67±10.00mg/dl) was significantly decreased. The calculated atherogenic index of plasma (AIP) was significantly higher (0.63±0.27) in post-menopausal women as compared to that in premenopausal women (0.50±0.29).Conclusion: Menopause leads to changes in lipid profile. By elevating LDL and the reduction of cardioprotective HDL is an indication that menopause is an independent risk factor for developing cardiovascular disease. These changes are caused by loss of cardio-protective effect of oestrogen.Anwer Khan Modern Medical College Journal Vol. 9, No. 1: Jan 2018, P 44-49
