BackgroundEarly development of neurocognitive functions in infants can be compromised by poverty, malnutrition and lack of adequate stimulation. Optimal management of neurodevelopmental problems in infants requires assessment tools that can be used early in life, and are objective and applicable across economic, cultural and educational settings.Objective and designThe present study examined the feasibility of infrared eye tracking as a novel and highly automated technique for assessing visual-orienting and sequence-learning abilities as well as attention to facial expressions in young (9-month-old) infants. Techniques piloted in a high-resource laboratory setting in Finland (N=39) were subsequently field-tested in a community health centre in rural Malawi (N=40).ResultsParents' perception of the acceptability of the method (Finland 95%, Malawi 92%) and percentages of infants completing the whole eye-tracking test (Finland 95%, Malawi 90%) were high, and percentages of valid test trials (Finland 69–85%, Malawi 68–73%) satisfactory at both sites. Test completion rates were slightly higher for eye tracking (90%) than traditional observational tests (87%) in Malawi. The predicted response pattern indicative of specific cognitive function was replicated in Malawi, but Malawian infants exhibited lower response rates and slower processing speed across tasks.ConclusionsHigh test completion rates and the replication of the predicted test patterns in a novel environment in Malawi support the feasibility of eye tracking as a technique for assessing infant development in low-resource setting. Further research is needed to the test–retest stability and predictive validity of the eye-tracking scores in low-income settings.

We report an electric-field poling study of the geometric-driven improper ferroelectric h-ErMnO 3 . From a detailed dielectric analysis we deduce the temperature and frequency dependent range for which single-crystalline h-ErMnO 3 exhibits purely intrinsic dielectric behaviour, i.e., free from extrinsic so-called Maxwell-Wagner polarisations that arise, for example, from surface barrier layers. In this regime ferroelectric hysteresis loops as function of frequency, temperature and applied electric fields are measured revealing the theoretically predicted saturation polarisation in the order of 5 -6 µC/cm 2 . Special emphasis is put on frequencydependent polarisation switching, which is explained in terms of domain-wall movement similar to proper ferroelectrics. Controlling the domain walls via electric fields brings us an important step closer to their utilization in domain-wall-based electronics.

Από τους 4.348 ασθενείς της πενταετίας 1986-1990 που νοσηλεύθηκαν στο Ουρολογικό Τμήμα του Ν.Γ.Ν.Θ. «Ο Άγιος Δημήτριος» η διάγνωση πάθησης στο ανώτερο ουροποιητικό τέθηκε σε 1201 και ραδιοϊσοτοπικό νεφρόγραμμα τελέσθηκε σε 214 ενώ το διουρητικό νεφρόγραμμα αξιολογήθηκε σε 66 από αυτούς. Οι 27 ήταν άνδρες και 39 γυναίκες. Το διουρητικό νεφρόγραμμα τελέσθηκε σε ασθενείς με: 1) παθήσεις πυελοουρητηρικής συμβολής, 2) λιθίαση ανωτέρου ουροποιητικού, 3) στενώματα ή Ca άνω τριτημορίου ουρητήρος, 4) ουρίνωμα (μετατραυματικό), 5) ετερόπλευρη ουρητηροϋδρονέφρωση λόγω πιθανής TBC ουροποιητικού, 6) κιρσοειδή διάταση παραουρητηρικών φλεβών, 7) χρονία νεφρική ανεπάρκεια αποφρακτικής αιτιολογίας, 8) TBC ουροποιητικού, 9) πολυκυστικούς νεφρούς, χρόνια πυελονεφρίτιδα και χρόνια νεφρική ανεπάρκεια εν γένει, 10) κάκωση νεφρών, 11) κυστεοουρητηρική παλινδρόμηση και μεγαουρητήρες και 12) ρικνούς νεφρούς. Επειδή όλες αυτές οι παθήσεις, παρά την πολυμορφία τους, έχουν τόσο κοινή εκδήλωση που είναι απαραίτητη η διαφορική διάγνωση, χρησιμοποιήσαμε το διουρητικό νεφρόγραμμα σαν πρόσθετο διαφοροδιαγνωστικό μέσο καθώς και στη μετά χειρουργική ή συντηρητική θεραπεία παρακολούθηση των ασθενών μας. Το ραδιενεργό νεφρόγραμμα αποτέλεσε το κριτήριο διαφορικής διάγνωσης της επιλογής θεραπευτικής αντιμετώπισης στην αποφρακτική ουροπάθεια, δηλαδή διαχώρησε τη διάταση με απόφραξη από την διάταση χωρίς απόφραξη (οργανική ή λειτουργική στένωση). Ταυτόχρονα αποτέλεσε την κύρια μέθοδο μετεγχειρητικής παρακολούθησης των ασθενών που είχαν υποστεί πυελοπλαστική. Θεωρήθηκε χρήσιμο συμπλήρωμα στην διαφορική διάγνωση, θεραπεία και παρακολούθηση των ασθενών με λιθίαση του ανωτέρου ουροποιητικού και με TBC ουροποιητικού. Σε όλες τις άλλες παθήσεις του ανωτέρου ουροποιητικού που χρησιμοποιήθηκε, αποτέλεσε διαγνωστικό βοήθημα ήσσονος σημασίας. Συμπερασματικά, το διουρητικό νεφρόγραμμα αποτελεί μία μέθοδο ανώδυνη, ακριβή και ακίνδυνη, μοναδική στη διαφορική διάγνωση της αποφρακτικής ουροπάθειας και χρήσιμη στις υπόλοιπες παθήσεις του ανωτέρου ουροποιητικού.
