A retrospective study of 104 patients (81 male and 23 females, the age range from 2 to 288days) with talipes equinus was conducted to explore the case factors associated with it. We analyzed and discussed the correlation of plaster correction times, age of first visit, gender, and birthplace of patients in the department and understood their correlation and causality. The data were analyzed using frequency analysis, normality test, chi-square goodness-of-fit test, chi-square test, and PLS regression. The findings are set out below. All the distributions of the number of plaster casts in the samples did not have normality. Therefore, we used the nonparametric test and partial least squares regression (PLS regression) and found that the number of plaster casts was more closely related to the age at first visit, gender, and birthplace and had a strong positive correlation. There was a negative correlation between the times of plaster correction and the compliance of braces. The lower the compliance of patients with braces, the more times the plaster correction will be conducted.

Complex clubfoot is a term used to describe a subset of patients that received previous treatment, and have distinctive anatomical features: severe equinus, with short first metatarsal, hyperextended big toe, severe plantar flexion of all metatarsals and deep folds through the sole of the foot and above the heel. Most complex clubfeet appear to be idiopathic and is usually associated with a poor casting technique. Complex clubfoot requires an early recognition and an adjustment of the casting protocol using the four finger Ponseti technique. This article gives the treating physician a general overview of the evaluation, treatment, and outcomes of complex clubfoot with the Ponseti method.

РЕЗЮМЕ. Вроджена клишоногість – поширена вада розвитку стопи в усьому світі, яка на початкових етапах комплексного лікування забезпечує високий відсоток позитивних результатів. Мета – загострити увагу практичних лікарів первинної ланки терапевтичного та хірургічного профілів на своєчасній діагностиці та адекватному лікуванні цієї тяжкої недуги. Матеріал і методи. Ретроспективний аналіз результатів сучасних науковців за останні десять років на ґрунті великого практичного матеріалу докорінно змінили тактику лікування цього небезпечного захворювання. Класичні методи лікування клишоногості на сучасному етапі в ортопедії дитячого віку практично не використовують, замінивши їх менш травматичним і ефективнішим методом Понсеті. Результати. Наведено приклад класичного способу лікування клишоногості з добрим результатом унаслідок ретельного оперативного та консервативного лікування протягом тривалого часу. Термін нашого диспансерного спостереження – 23 роки. Оперована ліва нога має невелику атрофію мускулатури стегна та гомілки в межах 2 см. Залишилося незначне приведення переднього відділу стопи з її поздовжнім вкороченням до 3 см. Спортсмен має вищу медичну освіту за спеціальністю «Фізична терапія, ерготерапія». Завдяки ретельній адекватній реабілітації досяг високих показників у спорті – чемпіон Європи (2020 р.) та світу (2023 р.) з пауерліфтингу. Висновки. 1. Класичний метод лікування вродженої клишоногості дає позитивний результат при ретельній довготривалій дороговартнісній реабілітації після оперативного лікування. 2. Наполегливість, стійкий характер, медична освіта молодого спортсмена сприяли повній реабілітації тяжкої вродженої клишоногості. 3. На сучасному етапі розвитку ортопедії доцільно застосовувати «золотий стандарт» лікування цієї недуги – метод Понсеті. 4. Позитивний успіх лікування можливий тільки при тісній співпраці лікаря, батьків і самої дитини.
