Correção cirúrgica de drenagem venosa pulmonar anômala total em adulto

Abstract: Total anomalous pulmonary venous drainage (TAPVD) is rarely seen in adults, because this congenital heart disease almost always requires surgical treatment in the neonatal period, often on an emergency basis. We report a patient that, despite being diagnosed during childhood, underwent surgical repair at age 25, about one year after his clinical condition worsened.

“…On an extensive search of the literature, there is limited data available regarding these patients who survive beyond infancy. The presence of large and non-restrictive ASD helps in the prevention of the development of pulmonary arterial hypertension which facilitates the survival of these patients beyond infancy [7][8][9][10]. The postoperative outcomes of these patients depend on the absence of post-repair pulmonary venous obstruction [11].…”

“…The mortality of which, when surgical repair is not performed, is estimated at 80% within the first year. 8 Few cases have been reported of a successful surgical correction in the second decade 8 and the sixth decade 7 but none in the Asian literature. The major factors that contributed to the long-term survival of our patient was the supra-cardiac type of TAPVD, unobstructed pulmonary venous drainage, a large inter-atrial communication, and maintenance of vascular resistance and pulmonary pressure closer to normal limits.…”
