Coronary vasospasm as the underlying cause for chest pain in patients with PVB19 myocarditis

Abstract: Coronary vasospasm is one of the main reasons for atypical chest pain in patients with clinical signs of myocarditis and biopsy-proven PVB19 myocarditis in the absence of significant CAD.

“…They also demonstrated that PVB19 infection tended to manifest like myocardial infarction, while HHV6 infection was likely to present with heart failure [11]. This notion was supported by a study with biopsy-proven myocarditis, where coronary artery vasospasm was more frequent in PVB19 myocarditis as compared with HHV6 myocarditis [9], and by reports that PVB19 preferentially invades vascular endothelial cells preferably, while HHV6 targets immune cells [12][13][14].…”

“…However, there has been uncertainty concerning the mechanism of coronary artery vasospasm associated with myocarditis and there are opinions that coronary vasospasm depends on the property of the invading virus [7][8][9].…”

“…Coronary spasm has been observed with this presentation in adults. 70 Parvovirus B19 has been found in the myocardium of such patients, as well as adenovirus and Epstein-Barr virus. 71 In a study of 4436 patients presenting to a pediatric emergency department with chest pain, 24 had a confirmed cardiac origin, of whom 4 were diagnosed with myocarditis.…”

“…Especially myocarditis caused by Parvovirus B19, which affects endothelial cells, has been associated with coronary vasospasm [2] . While coronary vascular smooth muscle cell dysfunction leading to Prinzmetal angina is an important differential diagnosis as well as coronary spasm on atherosclerotic coronary disease, myocarditis is an important but probably less frequent diagnosis to consider.…”
