Coronary Artery Obstruction After the Arterial Switch Operation for Transposition of the Great Arteries in Newborns

Abstract: Selective coronary angiography is the most accurate means to assess coronary artery obstruction after the arterial switch operation. Precise diagnosis of coronary artery lesions after this operation will help to elucidate the pathogenesis, develop adequate therapeutic strategies and might indicate how to prevent coronary complications after operation.

“…Nevertheless, we cannot assume that the problem disappears when childhood is over. The potential for silent myocardial ischemia in this setting, emphasis on the need for efficient screening for coronary complications, and routine coronary angiography has been suggested during adolescence and/or adulthood to be a part of the long-term follow-up [19] [20]. However, efficient screening is important, especially since patients with denervated hearts may not experience any symptoms of myocardial ischemia.…”

“…Mid-term follow-up is greatly influenced by coronary events after the Jatene procedure, as demonstrated in several studies (table III), where coronaryangiographies show them to occur in 3 to 11% of the cases during a 5-year follow-up after the surgery, with a mortality rate ranging from 0 to 10% [2][3][4][5][6] .…”
