Coronary Artery Disease in HIV-Infected Patients: Downside of Living Longer

Abstract: Purpose of Review Introduction of combination antiretroviral therapy (ART) has increased the life expectancy of patients with HIV infection, allowing them to live longer with this chronic medical condition and consequently experiencing conditions such as cardiovascular diseases (CVDs). Several studies have investigated the increased risk of CVD in people living with HIV (PLWH). However, less is known about the exact mechanisms involved in this increased risk. Also, specific guidelines for management of CVD in … Show more

“…As summarized above and in the companion article published with this paper [1], there is growing evidence indicating that HIV+ patients are at higher risk for most common causes of SCD, including CAD [15•, 20, 21••, 4045], cardiomyopathy and myocardial fibrosis [21••, 22, 46], heart failure [4, 5, 47], pulmonary hypertension [48], and some arrhythmias such as prolonged QT interval [49]. However, only a few studies have evaluated the risk of SCD in this population.…”

“…The mechanisms that are involved in the pathogenesis of CAD in PLWH, as detailed elsewhere [1], can also affect the extra-cardiac vascular system to a varying degree. However, the results of the limited available studies on the involvement of the vascular system have been less consistent than studies on CAD.…”

“…Cardiovascular disease (CVD) is the leading cause of death worldwide, representing an estimated 30% of all global deaths . This is of particular concern for people living with HIV (PLWH), who have been shown to be at an increased risk for developing atherosclerotic cardiovascular disease (ASCVD) compared with the general population . In high‐income countries, ASCVD is second to cancer as the leading cause of non‐AIDS‐related deaths for PLWH .…”

“…Hypertension is a leading risk factor for cardiac disease contributing to 9.4 million deaths annually and an independent predictor of mortality among HIV‐positive adults . Hypertension may be more prevalent among HIV‐positive individuals, due to antiretroviral use or because the virus itself is a risk factor for cardiovascular disease . In many resource‐limited settings, HIV and hypertension are managed separately without communication between providers.…”
