Background Prediction of severity or complexity of coronary artery disease (CAD) is valuable owing to increased risk for cardiovascular events. Although the association between total coronary artery calcium (CAC) score and severity of CAD, Gensini score was not used, it has been previously demonstrated. There is no information about the association between total CAC score and complexity of CAD. ObjectivesTo investigate the association between severity or complexity of coronary artery disease (CAD) assessed by Gensini score and SYNTAX score (SS), respectively, and coronary artery calcium (CAC) score, which is a noninvasive method for CAD evaluation in symptomatic patients with accompanying significant CAD. MethodsTwo-hundred-fourteen patients were enrolled. Total CAC score was obtained before angiography. Severity and complexity of CAD was assessed by Gensini score and SS, respectively. Associations between clinical and angiographic parameters and total CAC score were analyzed. ResultsMedian total CAC score was 192 (23.0-729.8), and this was positively correlated with both Gensini score (r: 0.299, p<0.001) and SS (r: 0.577, p<0.001). At multivariate analysis, it was independently associated with age (ß: 0.154, p: 0.027), male gender (ß: 0.126, p: 0.035) and SS (ß: 0.481, p< 0.001). Receiver-operating characteristic (ROC) curve analysis revealed a cut-off value > 809 for SS >32 (high SS tertile). ConclusionIn symptomatic patients with accompanying significant CAD, total CAC score was independently associated with SS and patients with SS >32 may be detected through high Agatston score.

Background: Coronary artery disease (CAD) represents a leading cause of death worldwide. Coronary angiography represents the gold standard for diagnosis and selection of the best treatment for patients with CAD. However some efforts have been made to predict CAD severity and complexity using non-invasive methods to identify the patients at high risk for cardiovascular events with less risk to the patients and before doing coronary angiography. Characterization of coronary-artery calcification by computed tomography known as Coronary artery calcium score (CACS) is proven to be equivalent to the total coronary atherosclerosis load and the angiographically significant lesions. Echocardiographic calcium score is now validated against non-coronary calcium by computed tomography with lower cost and no irradiation safety issues for reclassification of cardiac risk. Aim and Objectives: to determine the correlation of echocardiography calcium score and Aortic stiffness to severity of coronary artery disease. Patients and Methods: Patients coming to Ain Shams University Hospitals for elective coronary angiography were subjected to history taking, examination, blood samples and echocardiographic examination. The calculated echocardiographic calcium score and calculated Aortic stiffness were correlated with their coronary angiography films. Results: The study included 45 patients. The mean final calcium score of the whole study group was 4.95±1.29. The mean Aortic distensibility was 0.0044±0.0029. The mean syntax score of the whole study group was 22.88±12.3. There was highly significant difference between the numerical values of syntax score and final calcium score. There was weak negative but significant correlation between the syntax score numerical value and aortic distensibility. Conclusion: Echocardiographic calcium score is associated with the severity of CAD. The relationship between Aortic stiffness by echocardiography and severity of CAD still needs further evaluation. The low cost, availability and the radiation free nature of echocardiography make it an attractive candidate regarding the non-invasive tools for prediction of CAD.

SOUHRNKontext: Souvislost mezi tuhostí tepen (arterial stiffness, AS) a ischemickou chorobou srdeční (ICHS) již byla prokázána. V popisované studii jsme se pokusili zjistit vztah mezi různými parametry AS a rozsahem a závažností ICHS. Metody: Populaci ve studii tvořilo 411 pacientů s koronarografi cky potvrzenou ICHS. Měřili jsme různé parametry AS včetně augmentačního indexu (augmentation index, AIx), augmentačního tlaku (augmentation pressure, AP), rychlosti pulsní vlny (pulse wave velocity, PWV), centrálního systolického tlaku (central systolic pressure, cSys), centrálního diastolického tlaku (central diastolic pressure, cDia) a centrálního pulsního tlaku (central pulse pressure, cPP); zároveň jsme provedli analýzu pulsní vlny. K výpočtu Gensiniho skóre a Syntax skóre jsme použili angiogramy. Parametry AS byly porovnány s Gensiniho skóre a se Syntax skóre. Výsledky: Syntax skóre koreluje s věkem i s hodnotami cSys, cPP, PWV, AP, brachiálním pulsním tlakem (brachial pulse pressure, bPP), hemoglobinem, močovinou, přítomností diabetes mellitus, postižením kmene levé věnčité tepny (vždy p < 0,10). Gensiniho skóre však koreluje pouze s věkem, přítomností diabetes mellitus, postižením kmene levé věnčité tepny a bPP (vždy p < 0,10). Mnohorozměrová analýza prokázala, že věk, přítomnost diabetes mellitus, postižení kmene levé věnčité tepny a bPP významně predikují hodnotu Syntax skóre, zatímco v případě Gensiniho skóre byly za prediktory označeny věk, přítomnost diabetes mellitus, pohlaví, postižení kmene levé věnčité tepny a bPP. Závěr: Parametry AS nijak nesouvisejí s hodnotami Gensiniho skóre ani Syntax skóre. Kromě tradičních rizikových faktorů se jediným významným prediktorem v případě Gensiniho skóre nebo Syntax skóre zdá být bPP.
