Controversies in acute kidney injury: conclusions from a Kidney Disease: Improving Global Outcomes (KDIGO) Conference

Abstract: I n 2012, Kidney Disease: Improving Global Outcomes (KDIGO) published a guideline on the classification and management of acute kidney injury (AKI). 1 Since then, new evidence has emerged that has important implications for clinical practice. Large epidemiology studies and risk profiles for AKI have become available in adults and children, such as the AKI-Epidemiologic Prospective Investigation (AKI-EPI) study, 2 the 0by25 Initiative, 3 the Southeast Asia-AKI (SEA-AKI) study, 4 and the Assessment of Worldwide … Show more

“…Acute kidney injury was defined according to KDIGO, as an increase in serum creatinine of ≥0.3 mg/dl (26.5 μmol/l) during hospitalization 45 . Hypophosphatemia was defined as serum phosphate level <0.81 mmol/l and hypouricemia as a serum uric acid level below 2.5 mg/dl.…”

“…44 Acute kidney injury was defined according to Kidney Disease: Improving Global Outcomes (KDIGO), as an increase in serum creatinine of $0.3 mg/dl (26.5 mmol/l) during hospitalization. 45 Hypophosphatemia was defined as a serum phosphate level <0.81 mmol/l, and hypouricemia as a serum uric acid level below 2.5 mg/ dl. Inappropriate uricosuria and phosphaturia were defined as fractional excretion (FE) of uric acid >10% in patients with hypouricemia, or fractional excretion of phosphate (FE PO4 ) >20% in those with hypophosphatemia.…”

“…11 AKI was defined as an increase in serum creatinine to ≥1.5 times baseline, which is known or presumed to have occurred within the prior 7 days. 12 Oxaluria was reported as urinary oxalate-to-creatinine ratio. 4 , 13 Fecal acid steatocrit was recorded when available.…”

“…4 The KDIGO definition for AKI diagnosis has been adopted as a consensus established by experts worldwide. 4,13 Despite this, many Brazilian intensive care physicians continue to adopt other guidelines. 13 The incidence of AKI (21.3%) and mortality (25.7 %) in our study is in line with the meta-analysis conducted by the Acute Kidney Injury Advisory Group of the American Society of Nephrology, 4 with incidence and mortality of 21.6 and 23.9%, respectively.…”
