Controlled trial of penicillamine in severe rheumatoid arthritis.

Abstract: Heberden Society 385 abandoned but this change has taken place over the period that you mention. Could some of the improvement of results have been the abandoning of immobilization therapy ? PROF. DE BLECOURT Immobilization was not used. We saw in the beginning too many patients treated as cases of tuberculosis or immobilized for other reasons and completely crippled, and initially there was a marked shortage of physiotherapists, but nowadays we have quite a number of very skilled physiotherapists and physioth… Show more

“…In our present experience, prevalence of proteinuria in patients receiving BCL was 5.25% (21/400). This is a lower prevalence of proteinuria than that seen for D-PL (7.4–33%) [2, 3], which is structurally related to BCL. Characteristics of BCL nephropathy are quite similar to that of D-PL nephropathy, in that renal biopsy specimens show similar findings, prevalence of hematuria is similar, renal function is preserved despite massive proteinuria, and no significant relationship is evident between daily or cumulative dose and nephropathy [7].…”

“…Proteinuria, frequently accompanied by the nephrotic syndrome, is a common complication of treatment with antirheumatic agents such as D-penicillamine (D-PL) and gold, occuring in 1–12.5%, and 7.4–33% of patients, respectively [1,2,3]. Withdrawal of the drug consistently brings resolution of proteinuria.…”

“…X-ray photoelectron spectroscopy results (1) support a structure for the 1:2 complex in the solid state in which one penicillamine moiety acts as a monodentate carboxylate ligand and the second penicillamine is coordinated to zinc through the sulfhydryl and the amino groups. In solution, a structure in which the two penicillamine molecules act as bidentate ligands is more probable, the two sites of ligation being the sulfhydryl and the amino groups (2,11,14,23,28). The coordination could possibly involve the carboxylic group at lower pH range (6).…”
