PurposeThe purpose of this study was to describe the use of the nursing diagnosis Risk for Falls in Primary Care System of the Community of Madrid.MethodsA retrospective review of the clinical histories was carried out in 262 health centers from January 2005 to December 2015. The study population are the patients who have recorded in their electronic health record the nursing diagnosis Risk for Falls.FindingsFrequency of use of the Risk for Falls ND in the Community of Madrid was 53,340 diagnoses, increasing from 650 nursing diagnosis in 2005 to 14,695 in 2015. NOC Nursing Outcomes total identified were 109,145, which represents an average of 2.05 NOC Nursing Outcomes per diagnosis. NOC Nursing Outcomes frequently appeared as follows: Fall Prevention Behavior (35.9%), Safe Home Environment (11.3%), and Risk Control (10.5%). NIC Nursing Interventions total identified were 104,293, representing an average of 1.96 NIC nursing interventions per diagnosis. NIC Nursing Interventions frequently appeared as follows: Fall Prevention (45.9%), Environmental Management: Safety (27%), and Risk Identification (5.8%).ConclusionsNursing diagnosis of Risk for Falls and the care process related to this diagnosis is starting to be used by the primary care nurses of the Community of Madrid.Implication for nursing practiceRisk factors related to the nursing diagnosis of risk for falls identified in our study can be addressed with activities that nurses must implement to prevent falls. Nursing methodology in general and specifically the diagnosis of risk for falls must be included in guides and protocols for the prevention of falls, and its use should be promoted by primary care nurses.

Una las patologías más frecuentes en nuestro país Ecuador, como en todo el mundo, es la diabetes miellitus, que pertenece al grupo de las Enfermedades Crónicas no Transmisibles tales como hipertensión Arterial (HTA), enfermedad pulmonar obstructiva crónica (EPOC), cardiovasculares, artritis, artrosis, cáncer, dislipidemias, obesidad, entre otras, las cuales en conjunto están consideradas como un problema de salud pública, las mismas pertenecen a estadísticas importantes que determina que cada año aumentan en número las personas que las adquieren y cada vez la media de edad de presentación va disminuyendo. Alrededor de los 56 millones de defunciones ocurridas en el 2012, 38 millones son ocasionadas por enfermedades no transmisibles, de estos 28 millones ocurrieron en países de bajos recursos. Las causas más frecuentes son por: EPOC, cáncer, patologías cardiovasculares, diabetes, las mismas que se podrían evitar si se trabaja conjuntamente con prevención y promoción de la salud.

Maintaining an active lifestyle is a key health behavior in people with type 2 diabetes (T2D). This study assessed the feasibility and acceptability of a socio-ecological Nordic walking intervention (SENWI) to enhance healthy behaviors in primary healthcare settings. Participants included individuals with T2D (n = 33; age 70 (95% CI 69–74)) and healthcare professionals (HCPs, n = 3). T2D participants were randomly assigned to a SENWI, active comparator, or control group for twelve weeks. Feasibility and acceptability were evaluated based on a mixed methodology. Quantitative data reported adherence information, differences between follow-up and dropout participants and pre- and post-intervention on physical activity, sedentary behavior, and health outcomes. Qualitative data acquisition was performed using focus groups and semi-structured interviews and analyzed using thematic analysis. Thirty-three T2D invited participants were recruited, and twenty-two (66.7%) provided post-intervention data. The SENWI was deemed acceptable and feasible, but participants highlighted the need to improve options, group schedules, gender inequities, and the intervention’s expiration date. Healthcare professionals expressed a lack of institutional support and resources. Nevertheless, no significant difference between the SENWI follow-up and dropout participants or pre- and post- intervention was found (only between the active comparator and control group in the physical quality of life domain). Implementing the SENWI in primary healthcare settings is feasible and acceptable in real-world conditions. However, a larger sample is needed to assess the program’s effectiveness in improving healthy behaviors and its impact on health-related outcomes in the long term.
