Contrast Harmonic Power Doppler Imaging of Congenital Ventricular Diverticulum

Abstract: A case of an 81-year-old woman with a left ventricular diverticulum who underwent myocardial contrast echocardiography is reported. After administration of the contrast agent, a pulsed Doppler flow measurement clearly revealed the biphasic waveform of the ejection flow in the pre-systolic and systolic phase at the ostium of the diverticular cavity. A harmonic power Doppler image showed that part of the diverticulum wall had similar acoustic properties to the ventricular septal wall. Intracardiac blood flow and… Show more

“…Since it was first described by O'Bryan in 1866, 6 there have been less than 100 cases reported in English literature, their prevalence being estimated at 0.013% 7 . Most congenital ventricular diverticula were found in newborns and children, but it involved all age groups, as early as 24 hours after birth 3 and as late as 81 years of age 8 . It was also detected in fetal echocardiographic examination 9,10 .…”

“…Accompanying malformations can be assessed in the same setting and quantified by (color) Doppler flow measurements. Contrast harmonic power Doppler imaging provides information about contractility and myocardial perfusion of congenital left ventricular aneurysms or diverticula [26, 49, 89]. However, especially in adults, small apical changes can be missed due to difficult visualization of the left ventricular apex (fig.…”

“…In this situation, harmonic imaging may be useful. 8,12) In our first case, the diverticulum could be determined by conventional 2-D ECHO. Left ventriculography is more useful than magnetic resonance imaging or echocardiography for detecting this anomaly.…”
