Continued benefit of coronary stenting versus balloon angioplasty: five-year clinical follow-up of Benestent-I trial

Abstract: The original 10% absolute difference in TLR in favor of the stent group has remained unchanged at five years, emphasizing the long-term stability of the stented target site.

“…Although longer-term follow-up would have been enlightening, it seems unlikely that the cost-effectiveness of sirolimus-eluting stents would be less favorable with longer follow-up. Previous studies suggest that the restenosis process is largely complete after 12 months, 18 so later events are related predominantly to atherosclerosis progression, which would be unlikely to be altered by a drug-eluting stent. Moreover, exclusion of outpatient medication costs (except for clopidogrel) from our analysis may also have biased our analysis slightly against sirolimus-eluting stents.…”

“…This may represent no more than the play of chance, as the authors suggest. Only one other study (BENESTENT,114 ) comparing conventional balloon angioplasty and stents) reported details of deaths from cancer separately. Combining figures from these two RCTs confirms that the SOS result appears to be sustained (p = 0.002 on Fisher's exact test).…”

“…The few studies including longer-term follow-up of the firstgeneration Palmaz-Schatz stent also have focused on late stability of the target lesion. [1][2][3] Repeat revascularization of the target lesion resulting from restenosis is the most common event during this interval. The interest in restenosis events recently reached a crossroads with the advent of drug-eluting stents that have demonstrated their ability to reduce restenosis rates by roughly 70% compared with bare metal stents.…”
