Considerations for Implementing Point-of-Care Ultrasound in a Community-Based Family Medicine Residency Program

Abstract: Background:Prior to implementing Point-of-Care Ultrasound (POCUS) training into our Family Medicine Residency Program, we sought to determine opinions on the most relevant applications according to current Family Medicine residents and recent graduates. As there are so many POCUS applications relevant to Family Medicine, it would be efficient to teach only the most relevant scans. Objective:Examine current POCUS use and the perception of future use among current residents and recent graduates of a Family Medici… Show more

“…In accordance with our findings, another survey conducted by Weemer et al in a family medicine program reports that scanning a patient with biliary colic was perceived as a highly useful family medicine POCUS application by residents and graduates of the program [10]. AAA and the FAST POCUS applications were also ranked low in perceived usefulness.…”

“…Another randomized clinical trial demonstrated that a POCUS protocol for emergent patients significantly shortened the time to lifesaving treatment and also increased diagnostic accuracy while lowering mortality rates [ 9 ]. POCUS significantly lowers costs compared to ultrasound referrals [ 5 , 10 ]. While diagnoses are based on a patient's history, physical exam, and labs, the ready availability and diagnostic accuracy of POCUS means that clinicians will increasingly look within patients for definitive and expeditious answers [ 4 , 11 ].…”

“…[6][7][8] POCUS has utility for assessing patients, providing real-time diagnostic and monitoring information, and plays a key role in optimizing many procedures with direct visualization. [3][4][5]11,14 It is clear from this study that POCUS training across academic FM departments has dramatically expanded over the past 2 years, but significant barriers to clinical implementation continue to exist.…”

“…1 Recognizing the potential of POCUS in enhancing primary care delivery, [2][3][4] the American Academy of Family Physicians (AAFP) developed recommendations and guidelines for POCUS training and use in 2016. [5][6][7][8] However, several major roadblocks to widespread dissemination of POCUS into clinical care exist including lack of POCUS-trained faculty to teach a POCUS curriculum, 9 purchasing and storing equipment, training clinicians to use the technology and interpret images, 10 and understanding how to bill for clinical POCUS use. 11,12 There is a dearth of literature on how Family Medicine (FM) departments are faring on their journeys toward POCUS use in clinical care.…”
