Conservative treatment followed by chemotherapy with doxorubicin and ifosfamide for cervical sarcoma botryoides in young females

Abstract: Summary Sarcoma botryoides of the cervix is an extremely rare tumour and seems to be associated with a better prognosis than its vaginal counterpart. Recent studies have suggested that it is possible to limit surgery to local excision in stage I cases. We report three cases of young subjects treated successfully with polypectomy or diathermy loop excision followed by adjuvant chemotherapy. One patient had a local recurrence which was treated with further local excision. All subjects remain alive without eviden… Show more

“…Previous investigations of ovarian preservation for women with stage I epithelial ovarian cancer consisted primarily of small retrospective reports 15‐19. In those studies, the recurrence rate after fertility‐sparing surgery ranged from 4% to 28% 15‐17.…”

“…Previous investigations of ovarian preservation for women with stage I epithelial ovarian cancer consisted primarily of small retrospective reports 15‐19. In those studies, the recurrence rate after fertility‐sparing surgery ranged from 4% to 28% 15‐17. Most studies included stage IA and IC tumors, although fertility‐sparing surgery was reported in a small number of women with more advanced stage disease 15‐18.…”

“…However, while ultra-minimally-invasive procedures such as simple polypectomy for an exophytic, botryoid sarcoma have been reported, they carry the hazard of leaving behind residual gross or microscopic disease. Of three patients reported by Zanetta and colleagues who underwent simple polypectomy for cervical sarcoma botryoides, one suffered a local recurrence and a second was found to have an additional focus of disease in the endocervical canal [8]. Most recurrences of rhabdomyosarcoma of the female genital tract are locoregional [3].…”

“…8 Several other lesions enter into the pathologic differential diagnosis, including adenosarcoma, edematous cervical mesodermal polyps (pseudosarcoma botryoides), rhabdomyoma, and other tumors. 9 Adenosarcomas can also be found in young women but histologically can be distinguished from sarcoma botryoides by their fibrous stroma and leaf-like pattern of glands. The major surgical treatment for rhabdomyosarcoma which was performed in our patient consisted of complete resection of the primary tumor with a surrounding "margin" of normal tissue.…”

“…The prognosis becomes better especially when the tumor arises in a single polypoid lesion and is completely removed during surgery. 9 There are some prognostic factors defined for this tumor. Te majority of RMS cases occur sporadically and no predisposing factor is recognized for them.…”
