Characterization of falls in adults with established rheumatoid arthritis and associated factors

Background: Rheumatoid arthritis patients may have an increased risk of falls due to changes caused by the disease such as muscle weakness, joint impairment, reduced mobility and postural instability. The aim of this study was to prospectively analyze the occurrence of falls in RA patients and its risk factors. Methods: A cohort of 86 RA patients were assessed over 1 year for disease activity using the Disease Activity Score (DAS-28), for functionality using the Health Assessment Questionnaire (HAQ), for the characterization of falls and for the use of medications, and they were subjected to the Berg Balance Scale (Berg), Timed Up and Go (TUG), 6-Minute Walk (6MWT) and Short Physical Performance Battery (SPPB) tests. The Kolmogorov-Smirnov, Spearman's correlation, Student's t, Mann-Whitney and chi-square tests were performed with a significance level of P ≤ 0.05. Results: A total of 86 patients were evaluated, of which 48.8% had at least one fall and 75.6% reported having a fear of falling. No association of falls with age, disease duration, functional capacity, disease activity or physical performance was found. Patients with poorer performance in the physical tests had more functional impairment, higher disease activity and more advanced age. No differences in physical or functional performance, disease activity, gender or fear of falling were found between fallers and non-fallers; only a greater amount of medications used was found in the group of fallers. Conclusions: The occurrence of falls was high and associated with a previous history of falls and polypharmacy, with no association with disease activity or duration, functional capacity, physical performance, age or gender.

Inadequate production of saliva causes an oral imbalance that has a great impact on the individual's quality of life. Diseases of various types are responsible for impaired production of saliva and dry mouth, as happens in xerostomia. It is usually associated with hypofunction of the salivary glands, which shows signs of dryness on oral mucosa, and other morphological features in the oral cavity. Saliva is composed of water and 1% of electrolytes and immunoglobulins, enzymes and other proteins. Its fluid component, which contains ions, is mainly produced by parasympathetic stimulation, while the protein component, produced in the acinar secretory vesicles, is released by sympathetic stimulation. The use of certain drugs is a major cause of xerostomia. Among these we found analgesics, anticonvulsants, antihistamines, antihypertensives, diuretics and antidepressants. The treatment is mainly done by saliva substitutes such as sodium chloride, potassium chloride, monopotassium phosphate and dipotassium phosphate. However, in some cases, stimulants, such as citric acid and sodium citrate, or even topical salivary substitutes can be effective. Furthermore, the use of specific oral care products is well accepted by the patients. The treatment is selected according to the ability of the glands to produce saliva. It is still not fully known what mechanism of action drugs that cause hyposalivation, dry mouth or changes in saliva composition use. Yet, it is important to know the side effects of prescribed drugs.

RESUMOOs anticorpos anti-peptídeo citrulinado cíclico (anti-CCP), inicialmente descritos como fator antiperinuclear (APF) ou anticorpos anti-queratina (AKA), são classicamente descritos como autoanticorpos específicos da artrite reumatoide, inclusive sendo considerados como um dos marcadores sorológicos em seus critérios diagnósticos, elaborados pelo ACR-EULAR em 2010. No entanto, foram publicados pesquisas e relatos de caso demonstrando a ocorrência, em menor prevalência, em outras doenças autoimunes. Ao revisar a literatura médica, não foi encontrado nenhum estudo que reunisse todas os distúrbios autoimunes em que ele já foi avaliado ou descrito, tornando-se, portanto, o escopo da presente revisão sistemática. Utilizou-se o descritor "anti-CCP" na base de pesquisa Pubmed. Além da artrite reumatoide, foram encontrados 17 artigos abordando outros distúrbios autoimunes (artrite idiopática juvenil, artrite psoriática, psoríase, lúpus eritematoso sistêmico, esclerose sistêmica, doença de Behçet, síndrome de Jögren, rhupus). Destaca-se a importância de novos estudos serem conduzidos, prospectivos, com grupo-controle, e com significativo número de participantes, a fim de aumentar a evidência dos estudos encontrados.
