Congenital Midline Upper Lip Sinus

Abstract: The congenital upper lip sinuses are rare and they have previously been reported only in 40 cases. We have presented a case report of congenital midline upper lip sinus in an elderly age. Although the treatment modality is very easy and successful in any cases, the etiology of this rare congenital situation has been obscure and further studies should be done to find out.

“…The prevalence of CULS in the population is unknown and seems to be less than 0.00001%, which is the prevalence of lower lip sinuses (Fok et al, 2015). Delayed diagnosis in many cases of CULS is testament to the fact that asymptomatic CULS are underdiagnosed in infancy (Kriens et al, 1973; Mackenzie, 1970; Parisier and Birken, 1973; Sen et al, 2006).…”

“…However, variations in the locations of CULS were found since they were all located at a different length (upper, middle, or lower part of the philtrum). The upper location, just below the base of the columella, is the most frequent in the literature (Aoki et al, 2014; Asahina et al, 1997; Bakshi et al, 2015; Fok et al, 2015; Locke and Kubba, 2011; Rohart et al, 2015; Sen et al, 2006). An upper location is found in 65% of cases whereas lower and middle locations are, respectively, found in 25% and 10% of cases (Aoki et al, 2014; Asahina et al, 1997; Bakshi et al, 2015; Charrier et al, 2006; de Boutray et al, 2016; Sakamoto et al, 1992).…”

“…A range of hypotheses have been put forward to explain the pathogenesis of congenital midline fistula of the upper lip, including the fusion of facial prominences theory 10,19) , the merging of mesoblasts theory, and the invasion of abnormal epithelium theory 1) . A strong association has been suggested between midline cleft lip 4,11,15,18) and congenital midline fistula of the upper lip. One theory, in particular, suggests that midline fistula of the Congenital Midline Fistula of the Upper Lip upper lip is caused by planarization between the bilateral medial nasal processes due to a developmental problem of the front nasal prominence (medial nasal process) during the first 4-8 weeks of embryonic life, which would explain why fistulas are often formed at the midline and accompany midline cleft lip of the incomplete type due to problems occurring during the developmental process of the upper lip.…”
