Congenital aural atresia and stenosis: Surgery strategies and long-term results

Abstract: Meatoplasty with canalplasty and tympanoplasty in individuals with CAS can yield reliable and lasting positive hearing results with a low incidence of severe complications. The existence and preoperative condition of patients' TM and EAC skin helped improve hearing results and decrease the incidence of complications. However, the final hearing results and complications required stricter indications for CAA patients.

“…Consistent with several other series [5][6][7][8][9] with long-term followup, persistent and statistically significant hearing improvements have been found. Of note, these results reflect only the net change between the preoperative audiogram and the most recent postoperative audiogram.…”

“…In light of the technical simplicity and effectiveness of bone-anchored hearing devices, there are some who question the appropriateness of atresia repair because of its higher incidence of complications and less established long-term results. Several informative retrospective case series [5][6][7][8][9] evaluating these concerns have been reported, including a prior review of 45 patients (54 ears). 10 Nonetheless, atresiaplasty results remain a topic of interest.…”

“…Nearly all studies focus on postoperative stenosis and bony regrowth, but some studies neglect lateralization of TM. Lateralization of the TM was a common complication leading hearing loss in CAA, scar tissue contracture within the EAC graft would be one of risk factors [15]. In the correction of atresia, the prevention of complications is more beneficial than treatment.…”

“…Frequencies of 0.5, 1, 2 and 4 kHz were analysed in the present study. We collected the preoperative and postoperative PTA and calculated the air-bone gaps (ABG), and the mean of the four frequencies was calculated as the ABG value 9 . The short-term (< 1 yr) and long-term (> 1 yr) hearing results were reviewed to evaluate the stability of the hearing outcomes 29 .…”

“…Patients with stenosis sizes of 2 mm or less are at high risk for developing cholesteatoma and should undergo surgery; however, the data in the present study challenged this viewpoint. To date, there are no large sample studies focusing on the clinical features and long-term outcomes of CAS, and no studies have described the important parameter of measuring the diameter of EAC [5][6][7][8][9][10][11] . The aim of the present study was to comprehensively evaluate the clinical features and long-term outcomes of CAS, and challenge the previous viewpoint.…”
