Confluent and reticulate papillomatosis (Gougerot-Carteaud syndrome): a minocycline-responsive dermatosis without evidence for yeast in pathogenesis. A study of 39 patients and a proposal of diagnostic criteria

Abstract: CRP occurs predominantly in young adults and teenagers, with cosmetically displeasing brown scaling patches and plaques affecting the neck, upper trunk and axillae. Frequently, the diagnosis is delayed and the disorder not recognized by physicians, including dermatologists. Clinically, the eruption is most often confused with tinea versicolor. Potassium hydroxide staining of the scale is negative in the majority of cases, implying that fungi are not involved in the pathogenesis of this condition, as has been p… Show more

“…Histopathological examination reveals mild hyperkeratosis, papillomatosis, focal acanthosis and superficial perivascular lymphocytic infiltration. Although some treatment options such as salicylic acid, urea, topical and systemic retinoid, topical calcipotriol, tacrolimus, tazarotene, fluorouracil, antifungals, phototherapy and dermabrasion can be used, some studies of CRP performed in recent years have focused on the role of bacteria in the pathogenesis 1,[5][6][7] . Good response to antibiotics such as minocycline and azithromycin in clinical practice reinforces this theory.…”

“…insulin resistance), and genetic predisposing factors, have been speculated to be responsible for the pathogenesis of CRP, but none has been clearly shown to be causative, limiting evidence-based selection of therapeutic options. [2][3][4][5] In our case, the putative triggers have not been detected. Among the CRP cases reported in the last decade, just one case involved spontaneous regression, 3 but no detailed description of the clinical course was provided.…”

“…weigh loss) was noted in our case. 5 As most reported cases have been treated successfully with minocycline and followed up only for a short period, 3,4 it is possible that CRP may be a transient condition for which spontaneous regression can be expected and active treatment may not be mandated unless cosmetically required. Further accumulation of CRP cases is necessary to draw definitive conclusions.…”
