Diabetes is the most common metabolic disease. Type 2 diabetes is a variable combination of insulin resistance and disorder in insulin secretion, leading to disorder of lipids and plasma lipoproteins. The most common pattern of dyslipidemia in diabetic is high triglyceride (TG) and low high-density lipoprotein cholesterol (HDL-C). This study was conducted to find a more effective drug to increase HDL-C. In this study, 80 patients (26 males and 54 females) with type 2 diabetes received fenofibrate in cross-sectional way for 2 months, and they did not take antilipid drugs for 2 month. Then, they underwent atorvastatin for 2 months and HDL-C was measured before and after taking drugs. Patients did not change their diet during this study. Effect of atorvastatin and fenofibrate on HDL-C levels in patients with type 2 diabetes was evaluated. The mean HDL-C and total cholesterol (TC) before and after taking drugs were 36.5 mg/dL and 174.56 mg/dL, respectively. After atorvastatin, the mean HDL-C and TC were 43.30 and 150.144 mg/dL, respectively, and after fenofibrate, 43.40 were mg/dL and 146.36 mg/dL, respectively. Atorvastatin caused increase in HDL-C by 18.44% and reduction in TC by 13.82% and fenofibrate increase in HDL-C by18.62% and reduction in TC by 16.05%. No difference was seen between atorvastatin and fenofibrate in terms of effect on the HDL-C excess (P = 0.449). In addition, no difference was seen between atorvastatin and fenofibrate in terms of effect on TC reduction (P = 0.992). In conclusion various factors are involved in increasing the HDL, such as race, sex, nutrition, physical activity and, of course, medications. The effect of medications is also different on races and genetics. The value of increase in HDL-C after Fenofibrate and Atorvastatin was associated with gender so that it caused more increase of HDL-C in females.

У світі спостерігається підвищення частоти гестаційного цукрового діабету (ГЦД) на тлі різкого зростання надмірної маси тіла та ожиріння. Гестаційний цукровий діабет має негативні найближчі та віддалені наслідки для здоров’я жінок та їхнього потомства. З’ясовано роль кількох чинників ризику розвитку ГЦД, таких як ожиріння, наявність цукрового діабету 2 типу в родині, вік матері. Однак значення інших чинників ризику остаточно не визначено. Оскільки ГЦД асоціюється з посиленням резистентності до інсуліну в периферичних тканинах, розглядають можливу участь дефіциту вітаміну D у патогенезі ГЦД. Мета роботи — дослідити взаємозв’язок між дефіцитом вітаміну D у сироватці крові впродовж першого триместру вагітності та ризиком розвитку ГЦД в третьому триместрі. Матеріали та методи. Проведено проспективне дослідження за участю 68 вагітних, обстежених у першому триместрі (6 — 13 тиж вагітності), з них 52 взяли участь у скринінгу ГЦД у третьому триместрі (24 — 28 тиж вагітності). Визначали рівень вітаміну D у сироватці крові, біохімічні, антропометричні параметри. Використовували метод логістичної регресії для аналізу зв’язків між дефіцитом вітаміну D і виникненням ГЦД. Результати. Дефіцит вітаміну D (< 20 нг/мл) у першому триместрі зареєстровано у 42 (61,8 %) вагітних. У третьому триместрі в 14 (26,9 %) вагітних діагностовано ГЦД за результатами перорального тесту на толерантність до глюкози, з них у 11 вагітних, які мали дефіцит вітаміну D у першому триместрі. Ризик ГЦД був статистично значущо вищим у жінок з дефіцитом вітаміну D порівняно з вагітними з нормальним його рівнем (відносний ризик — 6,67; p < 0,001, 95 % довірчий інтервал — 3,56 — 15,57). Цей ризик значно підвищувався після внесення поправок на вік, кількість вагітностей, величину індексу маси тіла, наявність цукрового діабету у родині (відносний ризик — 10,4; p < 0,001, 95 % довірчий інтервал — 2,82 — 39,76). Висновки. Дефіцит вітаміну D у першому триместрі статистично значущо збільшує ризик виникнення ГЦД.

A systematic review and meta‐analysis of the effect of Vitamin D‐fortified food on glycemic indices

Some reports indicated that Vitamin D may improve glycaemia indices in diabetic patients. The aim of this systematic and meta‐analysis was to evaluate effects of Vitamin D fortification on indices of glycemic control. Six databases (PubMed/Medline, ISI Web of Knowledge, Cochrane Library, Science Direct, Scopus, and Google Scholar) were searched, for randomized controlled trials that were published up to September 2018 and that compared the effect of Vitamin D‐fortified food versus regular diet in relation to glycemic control. Of the 4,379 studies originally found, 11 articles remained to be assessed for meta‐analysis. Vitamin D fortification was associated with a significant improvement in fasting serum glucose (mean difference [MD]: −2.772; 95% confidence interval [CI]: −5.435 to −0.109) and fasting serum insulin (MD: −2.937; 95% CI: −4.695 to −1.178) in patients with Type 2 diabetes mellitus. A diet with food enriched with Vitamin D was associated with a significant improvement in homeostatic model assessment of insulin resistance (MD: −1.608; 95% CI: −3.138 to −0.079) but was not associated with a significant reduction in hemoglobin A1C (MD: 0.034; 95% CI: −0.655 to 0.069). This meta‐analysis indicates that Vitamin D fortification improves indices of glycemic control. Hence, food fortified with Vitamin D may be of potential therapeutic value in diabetic patients, as an adjuvant therapy.
