Human polymorphonuclear neutrophils (PMN) chemotax to a foreign entity. When the chemoattractants' origins are reached, specific receptors bind to the invader's surface, initiating phagocytosis, phagosome formation, and fusion with granule membranes, generating the bactericidal oxidative burst, and releasing lytic enzymes, specific peptides, and proteins. We explored the initial signaling involved in these functions by observing naïve, unprimed PMN in suspension using fluorescent indicators of cytoplasmic signals (⌬[Ca 2؉ ] i and ⌬pH i ) and of bactericidal entities (oxidative species and elastase) exposed to N-formyl-methionyl-leucylphenylalanine (fMLP) and/or multivalent immune complexes (IC Human polymorphonuclear neutrophil (PMN) activation is triggered by the liganding of the stimulus to its specific receptors (4,8,29,30,(40)(41)(42)(43). Chemoattractants released by an organism or by the opsonins that coat it, such as N-formylmethionyl-leucyl-phenylalanine (fMLP), are recognized by their PMN surface receptors. Naïve PMN have been shown to possess two classes of fMLP receptors (fPR1 and fPR2, of high and low affinity, respectively) (25, 40-43), which, when liganded with up to 100 nM peptide, mediate the activation of signals, including a rapid transient rise in cytoplasmic Ca 2ϩ ([Ca 2ϩ ] i ), due largely to release from intracellular stores, and a simultaneous drop in cytoplasmic pH (pH i ) (14,19,26,27,37); when the fMLP concentrations are above 1 M, the loweraffinity fMLP receptor also initiates some release of bactericidal entities (7,25,42,43,49). The initial rapid cytoplasmic signals initiated by fPR1 have been studied extensively; they are followed within less than 1 min by a redistribution such that a final [Ca 2ϩ ] i up to 100 nM above the resting [Ca 2ϩ ] i ([Ca 2ϩ ] i 0 ) of naïve PMN as well as a pH i 0.2 to 0.3 units higher than its resting pH i (pH 0 ) of 7.05 to 7.07 are attained (2, 15, 37). These changes, in turn, cause the PMN, after cytoskeletal rearrangement (15,33,37) and expression of adhesion proteins on their surface, to move (chemotax) up the chemoattractant gradient to its source via multiple contacts between a given PMN and higher fMLP concentrations (32), a motion that has been captured in real time (23).Many investigations have shown that at or below 100 nM, liganding with fMLP does not induce the activation of the bactericidal functions of human PMN (e.g., oxidative burst and lytic enzyme and inhibitory protein release) unless the PMN have been primed (3,6,13,15,39,48,50). ÖzetDişeti oluğu sıvısı (DOS) kendine özel yapısı nedeniyle periodontal sağlık ve hastalık hakkında bilgi verme potansiyeline sahiptir. Bu yapısı nedeniyle sıvının teşhis basamağında önemli veriler sunabileceği düşünülmektedir. DOS örneklerinin elde edilmesi girişimsel olmayan ve objektif değerlendirmeye izin veren bir süreç olması, bölgesel düzeyde incelemelere olanak vermesi tanısal potansiyelinin önemli bir unsurunu oluşturmaktadır. DOS'un elde edilmesinde kullanılan değişik yöntemler vardır. Her değerlendirme yönteminin kendisine ait avantaj ve dezavantajları mevcuttur. Günümüzde henüz kabul edilmiş ideal DOS örnekleme protokolü mevcut değildir. Son yıllarda ideal bir DOS örnekleme stratejisinin geliştirilmesine yönelik önerilere literatürde sıklıkla rastlanmaktadır. Birçok çalışmada örneklemelerin elde edilmesi sürecinde hacimsel ve içerik değişimlerine neden olabilecek bazı potansiyel faktörlerin gözetilmesi gerekliliğinin altı çizilmektedir. Bu faktörlerden bazıları ana hatlarıyla; DOS elde edilmesi ve hacminin belirlenmesi sürecinde dikkat edilmesi gereken hususlar ve çevresel (buharlaşma, oda sıcaklığı-nem vb.) faktörlerdir. Bu derlemenin amacı DOS'nın elde edilmesi ve değerlendirme sürecinde metodolojik açıdan, sıvıya etki edebilecek potansiyel faktörleri (kısmen veya tamamen kontrol edilebilen-edilemeyen) ele almak ve sistematik olarak irdelemektir. Background: Curcumin has anti-inflammatory properties. The aim of this study was to compare interleukin-1b (IL-1b) and chemokine (C-C motif) ligand 28 (CCL28) levels following a topical application of curcumin (CRM), chlorhexidine (CHX) and chlorhexidine-metronidazole (CHX-MTZ) in an experimental gingivitis human model. Methods: Sixty systemically healthy selected subjects were randomly assigned to one of three topical antigingivitis gels. Each gel was applied twice daily for 10 minutes as the sole method of oral hygiene for 29 days on the test quadrant only. Modified gingival index (MGI), plaque index (PI), bleeding on probing (BOP) and probing depth (PD) were assessed at baseline, 29 days and 60 days. Estimation of IL-1b and CCL28 levels in gingival crevicular fluid was done at baseline and at 29 days. Results: The increase of IL-1b in the CRM (14.52 AE 16.6 pg/ml) and CHX-MTZ (31.63 AE 15.96) groups was significantly less than that of the CHX group (70.55 AE 38.81). Similar results were also observed for CCL28 (CRM: 8.12 AE 8.78 pg/ml; CHX-MTZ: 12.81 AE 18.68; CHX: 41.15 AE 22.82). All groups had a significant increase in MGI, PI and BOP at 29 days. Conclusions: The anti-inflammatory potential of topical curcumin was similar to CHX-MTZ but superior to CHX in affecting IL-1b and CCL28 levels.
