Worldwide, HIV prevention is challenged to change because clinical trials show the protective effect of technologies such as circumcision, preexposure prophylaxis, and the suppression of viral load through antiretroviral treatment. In the face of demands for their implementation on population levels, the fear of stimulating risk compensation processes and of increasing riskier sexual practices has retarded their integration into prevention programs. In this article, following a narrative review of the literature on risk compensation using the PubMed database, we offer a critical reflection on the theme using a constructionist approach of social psychology integrated to the theoretical framework of vulnerability and human rights. The use of biomedical technologies for prevention does not consistently induce its users to the increase of riskier practices, and variations on the specificity of each method need to be carefully considered. Alternatives to the theories of sociocognitive studies, such as social constructionist approaches developed in the social sciences and humanities fields, indicate more comprehensive interpretations, valuing the notions of agency and rights. The critical analysis suggests priority actions to be taken in the implementation process: development of comprehensive programs, monitoring and fostering dialog on sexuality, and technical information. We highlight the need to implement a human rights-based approach and to prioritize dialog, stressing how complementary these technologies can be to meet different population needs. We conclude by stressing the need to prioritize sociopolitical changes to restore participation, dialog about sexuality, and emphasis on human rights such as core elements of the Brazilian AIDS policy.

Analysis of deaths due to acquired immunodeficiency syndrome

Objective: to analyze the profile of the deaths of people with acquired immunodeficiency syndrome. Methods: a cross-sectional study was carried out with 106 medical records of people who died in an intensive care unit, obtaining sociodemographic and clinical variables. Analysis made using Pearson's chi-square test, Fisher's test and Mann-Whitney test. Results: most of the patients were male (80.2%), in the age group from 18 to 39 years, average age of 39.7 years, 81.6% dark-skinned, 60.4% were single and 62.5% unemployed. The proportion of patients who were unemployed (p=0.002) and who did not use antiretroviral therapy (p<0.001) were the ones that had the least regular health care. Conclusion: most of the patients in the study did not follow regular health care and did not use antiretroviral therapy. Descriptors: Acquired Immunodeficiency Syndrome; Death; Health Promotion. Objetivo: analisar o perfil dos óbitos de pessoas com síndrome da imunodeficiência adquirida. Métodos: estudo transversal, realizado com 106 prontuários de pessoas que foram a óbito em unidade de terapia intensiva, obtendo-se variáveis sociodemográficas e clínicas. Análise realizada através do teste qui-quadrado de Pearson, teste de Fisher e teste de Mann-Whitney. Resultados: maior parte dos pacientes era do sexo masculino (80,2%), na faixa etária de 18 a 39 anos, média de idade de 39,7 anos, 81,6% de cor parda, 60,4% solteiros e 62,5% desempregados. A proporção de pacientes desempregados (p=0,002) e que não faziam uso de terapia antirretroviral (p<0,001) foram os que menos realizavam acompanhamento regular de saúde. Conclusão: a maioria dos pacientes do estudo não realizava acompanhamento regular de saúde e não utilizava terapia antirretroviral.

Scientific Production about the Adherence to Antiretroviral Therapy

Objective To identify the elite of authors about the subject adherence to antiretroviral therapy; to identify the journals turned to publishing articles about adherence to antiretroviral therapy; and to identify and analyze the most commonly used words in abstracts of articles about adherence to antiretroviral therapy. Method A bibliometric study conducted through the Scopus base. We used articles published between 1996 and 2014, after application of the eligibility criteria, there were composed the sample with 24 articles. The data were analyzed descriptively. Were used the laws of bibliometric (Lotka, Bradford and Zipf) and the conceptual cloud map of words, through the program Cmap tools. Results Lotka’s Law identified the 5 authors more productive (46% of the total published). Bradford is impaired in this study. Concerning Zipf, 3 zones were determined, 31.47% of the words with in the first zone, 26.46% in the second and 42.06% in the third. In the conceptual map, the words/factors that positively and negatively influence adherence were emphasized, among them the need for more research in the health services. Conclusion There are few publications about the accession to antiretroviral therapy, and the scientific production is in the process of maturation. One can infer that the theme researched is not yet an obsolete topic. It should be noted that the Bibliometric was a relevant statistic tool to generate information about the publications about the antiretroviral therapy.
