The objectives of this investigation were to study the respiratory tract colonization and transmission of enterococci between 20 patients treated with mechanical ventilation at an intensive care unit (ICU), to compare genotyping with phenotyping, and to determine the antibiotic susceptibilities of the isolated enterococci. Samples were collected from the oropharynx, stomach, subglottic space, and trachea within 24 h of intubation, every third day until day 18, and thereafter every fifth day until day 33. Enterococcal isolates (n ‫؍‬ 170) were analyzed by pulsed-field gel electrophoresis and with the PhenePlate (PhP) system. The antimicrobial susceptibilities to five agents were determined. Seventeen of the 20 subjects were colonized with enterococci in the respiratory tract; 12 were colonized in the lower respiratory tract. Genotype analyses suggested that 13 patients were involved in a transmission event, including all patients intubated more than 12 days. In conclusion, colonization of resistant enterococci in the respiratory tract of intubated patients treated at an ICU was common. Transmission of enterococci between patients occurred frequently. Prolonged intubation period seems to be a risk factor for enterococcal cross-transmission.

. 1997. We used the Phene Plate generalized microplates to investigate the metabolic activities of faecal flora of pigs during pre-and post-weaning periods. Weekly samples were collected from four sows and their litters (four piglets from each) during 5 months. The metabolic fingerprints obtained from faecal floras of sows and their litters in the first sampling occasion was very similar, suggesting that sows were the initial source of flora for piglets. This similarity, however, was lost in week 2 and piglets developed new types of flora which, although similar among the litter-mates, differed from those of the sows. The metabolic fingerprints of pigs' floras during the post-weaning period also differed from those of the suckling period. On day 70 pigs were transferred to a fattening stable. The faecal flora of the animals during this period was unstable in each individual and differed among litter-mates. A pattern of successive changes was observed in the fermentative capacity (FC) of pigs' floras reaching the highest value before weaning (day 34). An overall decrease in the FC value of faecal floras was observed as a consequence of dietary shifts from milk to solid food to high energy fattening diet. The mean FC value of pig floras on the first sampling occasion (mean 2 S.D. 0·41 2 0·02) was significantly higher than that obtained in the last sampling occasion (day 145) (0·31 2 0·04) (P ³ 0·001). Faecal floras of sows also had lower FC values (0·25 2 0·01) than those of piglets suggesting that loss of FC by the faecal flora of young pigs will continue as they age.

Sparse information is available on the virulence factors of Aeromonas strains isolated from diseased fish, from the environment, and from humans. In the present study, 52 Aeromonas isolates obtained from epizootic ulcerative syndrome (EUS) lesions in fish, from the aquatic environment, and from children with diarrhea in Bangladesh were identified by biochemical phenotyping (i.e., PhenePlate [PhP] typing) and DNA fingerprinting and then characterized with respect to certain putative virulence factors. The isolates from the fish exhibiting EUS symptoms were identified to be Aeromonas veronii biovar sobria by fatty acid methyl ester analysis and amplified fragment length polymorphism fingerprinting. Biochemical phenotyping revealed that all EUS-associated isolates belonged to a unique phenotype which was not identified among more than 1,600 environmental and diarrheal isolates in a previously collected database of PhP types of Bangladeshi Aeromonas isolates. The 52 Aeromonas isolates were investigated for the production of hemolysin and cytotoxin; for hemagglutination with erythrocytes from fish, human, and rabbit sources; for the presence of a cytolytic enterotoxin gene; and for adhesion to and invasion into fish cell lines. All of the EUS isolates produced all of the virulence factors investigated, as did also some of the environmental isolates, but the isolates from EUS were unique in their ability to agglutinate fish erythrocytes. Our results suggest that a clonal group of A. veronii biovar sobria is associated with, and may be a causative agent of, EUS in fish in Bangladesh.
