Background Traditionally, surgeons rely on manual measurements to translate pre-surgical plans into clinical reality during sinus floor elevation with a lateral approach. Standardizing this surgical phase could offer a reproducible technique minimizing operational errors and ensuring more consistent and predictable outcomes. Objective This case report presents a comprehensive digital approach integrating lateral antrostomy (LAIDA) for the elevation of the maxillary sinus floor. Case Presentation Lateral sinus floor elevation was required for a 61-year-old patient. A detailed digital protocol, integrating artificial intelligence, was employed to create a tooth-supported surgical guide for antrostomy. Under local anaesthesia, a full-thickness flap was elevated to expose the lateral sinus wall. The surgical guide was accurately positioned, and the antrostomy area was marked on the bone wall. The bony window was then performed using round diamond burs, and the Schneiderian membrane was gently lifted until the medial wall of the sinus was exposed. The sub-antral space was filled with a composite graft, and three implants were placed using the same surgical guide. The antrostomy was covered using a resorbable collagen membrane, and flaps were sutured with synthetic monofilament. No intra- and post-operative complications were recorded. Conclusion Accurate antrostomy planning and execution are crucial, considering factors, like sinus floor location, presence of alveolar-antral artery, and Underwood septa. The present study emphasizes the benefits of a standardized approach using a 3D-printed surgical guide, aiding real-time visualization of the predetermined bony window during surgery. The LAIDA workflow may help in minimizing intra-operative complications, enhancing patient comfort and improving surgical outcomes.

Aim To validate in vitro the accuracy of a novel method for bone augmentation of horizontal alveolar bone defects with the help of a surgical guide. Material and methods Six cone‐beam computed tomography scans of patients requiring horizontal bone augmentation were segmented and 3D printed. Two surgeons performed the bone augmentation surgery twice for each case on 3D‐printed models, using either a conventional or guided protocol. Each surgeon virtually planned the desired graft shape beforehand. The resulting grafts were compared linearly and volumetrically to the plan; graft density and surgical time were also analyzed. Results There was significantly less graft volume outside the planned volume with the guided protocol (36.8% ± 14.1 vs. 19.6% ± 12.3, p < .01). The use of a guide increased graft accuracy at several measurement points, resulting in less overfill when using the guided protocol (1.7 ± 1.7 mm at the most coronal point, 0.2 mm ± 1.4 at 25%, 0.0 mm ± 0.9 at 50%, 0.1 mm ± 1.1 at 75%, and 0.4 mm ± 1.4 apically). Graft thickness was increased 1 mm coronally from the planned graft (2.8 mm ± 2.3 vs. 1.8 mm ± 2.2, p = .04). Surgical time increased significantly when using a guide (a difference of 2 m 26 s). Conclusion The technique, tested here in vitro, allows surgeons to improve the accuracy of the resulting graft shape, to better compact material in the coronal portion of the graft, and to reduce the amount of graft material placed outside the planned volume.

This article describes a technique for performing lateral antrostomy when performing lateral sinus augmentation using a rational approach. Based on a CBCT scan, a surgical guide is designed and manufactured to allow the surgeon to draw the antrostomy flush with the anterior sinus wall and floor, with its height not exceeding 6 mm. The distal antrostomy line is placed in relation to the position of the most distal fixture. This technique facilitates sinus membrane detachment and reduces flap elevation to approximately 10 mm, and potentially prevents intraoperative and postoperative complications saving the patient from later discomfort. The technique is easily reproducible and because it involves a series of standardized steps, it reduces the likelihood of intraoperative errors.
