Outcome Analysis of Percutaneous Vertebroplasty in Osteoporotic Compression Fractures

Introduction: Osteoporotic compression fracture of vertebra are very commonly occurring with increasing age. Pain and deformity are most common features. Vertebral augmentation procedures like percutaneous vertebroplasty is a common surgical modality used for osteoporotic compression fractures. Materials and Methods: This prospective observational study was conducted on 36 patients of vertebral compression fracture that underwent percutaneous vertebroplasty in Datta Meghe Medical College in association with Jawaharlal Nehru Medical College. A thorough clinical assessment including physical, neurological, and systemic examination was carried out before taking up patients for percutaneous vertebroplasty. Bony Tenderness at fracture site was examined. Results: This study was conducted in 36 patients of vertebral compression fractures at our institute from January 2019 to January 2020. In our study, out of 36 patients, 24 (66.6%) were females and 18 (33.3%) were males. The mean age was 56.5 ± 6.2 SD years. D12 vertebral fracture was most commonly involved vertebra. The VAS score was used to assess pre and post procedure improvement. The mean pre-operative score 7.4 ± 1.1 SD which reduced to 2.2 ± 0.8 SD at 1 month followup and 2.1 ± 0.5 SD at 1 year follow up. There was significant reduction in pain score post operatively when compared with pre-operative scoring (p value < 0.01). Conclusion: On the basis of this study, we concluded that vertebroplasty is a safe, cost-effective, and pain relieving treatment option available for geriatric patient with osteoporotic vertebral compression fractures which are not relieved by conservative management.

Background: There is a very high morbidity and mortality rate in patients who have intra cerebral hemorrhage. The ICH Score is a very good predictor of outcome of 30-day mortality as found in multiple studies. The ICH Scores range from 0 to 5 and higher Score is associated with increased mortality after 30 days and thus it was found to be a good predictor of outcome. So it was decided to apply it in the assessing of patients with intra cerebral hemorrhage and to study it's outcome. Aim: To study the ICH score utility in management of patients with intracerebral bleed Objectives: To improve the quality indicators of Emergency Department, to cater the right management at right time in patients with intracerebral bleed and Inclusion of ICH score in the assessment sheet for screening. Materials and Methods: In the study, the patients brought to Emergency Department who are diagnosed to have ICH on CT Brain will be evaluated for GCS, Age, Infratentorial origin, volume of the ICH, absence or presence of Intraventricular Hemorrhage to determine ICH score and outcome of the patients. Expected Results: Patients having lower ICH score are expected to have poorer prognosis. Inclusion of ICH score in assessment of patients with ich is expected to provide better treatment for these patients.

Background: Clival canal angle is an angle formed by the clivus and the posterior longitudinal ligament of the cervical spine and Welcher basal angle formed between the line joining the nasion to tuberculumSella and the line joining the tuberculum sellae to basion along the plane of clivus. With the aim to review the normal distribution of clival canal angle and basal angle in the Nepalese population this study was performed. Methods and materials: This is a cross-sectional analytical study with non- probability consecutive sampling technique done over the duration of 3 months. A reconstructed image of a bone window in the midsagittal plane was selected and measurement of the angles was done in CT console. Results: There was a total of 60 patients in this study with male predominance (figure 3). The mean age of the population was 49.8 (SD 18.8) years. The clival canal angle reference range was 130.8 to 168.8 degrees with a mean 149.8(SD 9.5) degree. Similarly, the reference range of Welcher basal angle in the study population was 115.49 to 140.29 degree with a mean of 127.89(SD: 6.4) degrees. Conclusion: The mean clival canal angle in the Nepalese population is 149.8 (SD: 9.5) degrees and the mean basal angle is 127.89(SD: 6.4) degree.
