SUMMARY Prosthetic joint infection (PJI) is a tremendous burden for individual patients as well as the global health care industry. While a small minority of joint arthroplasties will become infected, appropriate recognition and management are critical to preserve or restore adequate function and prevent excess morbidity. In this review, we describe the reported risk factors for and clinical manifestations of PJI. We discuss the pathogenesis of PJI and the numerous microorganisms that can cause this devastating infection. The recently proposed consensus definitions of PJI and approaches to accurate diagnosis are reviewed in detail. An overview of the treatment and prevention of this challenging condition is provided.

The elbow joint is a complex joint, which, when impaired in function, leads to severe disability. In some cases however, an arthroplasty might be an appropriate treatment. In the past four decades, large steps have been taken to optimize this treatment in order to achieve better post-operative outcomes. To understand these progresses and to discover aspects for upcoming improvements, we present a review on the past developments, the present state of affairs and future developments which may improve patient care further.

Anatomi-MekanikDirsek eklemi mekanik olarak vücuttaki hareketli eklemlerin en stabil olanıdır. Dirsekte üç eklem mevcut olup iki farklı hareket vardır.Ulnohumeral eklem menteşe benzeri yapı gösterir ve fleksiyon ve ekstansiyon hareketlerinden sorumludur. Radiohumeral ve proksimal radioulnar eklemler ise aksiyal rotasyondan sorumludurlar. Önkol tam ekstansiyonda iken dirsek aksı 0 derece ve tam fleksiyonda iken yaklaşık 146 derecedir. Dirsekte aktif ve pasif hareket açıları arasında kas tonusuna bağlı olarak ortalama 5 derece kadar fark vardır. Aktif hareket açıklığı fleksör kasların kol ve önkol arasındaki apozisyonu nedeniyle pasif harekete göre daha sınırlıdır. Dominant dirsek ile non-dominant dirsek arasında aktif hareket açıklığı açısından anlamlı fark bulunamamıştır. Bu nedenle hareket açısındaki kayıplar diğer dirsekle mukayese edilerek değerlendirilir. Şişman ve kaslı insanlarda hareket açıklığında 10 dereceye kadar azalma gözlenirken çocuklarda ve bayanlarda erişkin ve erkeklere göre 5-8 derece kadar hiperekstansiyon görülebilmektedir [1,2,3]. DİRSEK OSTEOARTRİTİDirsek osteoartriti sık görülmeyen ancak ciddi eklem ağrısına, dirsekte kilitlenmeye ve dirsek hareket kaybına yol açabilen dramatik bir hastalıktır.Genel populasyonda görülme oranı %2-3 olup erkek populasyonda görülme sıklığı kadınlara göre 4-5 kat daha fazladır. Özellikle ağır iş yapan orta yaş erkek grupta daha sık görülür [4,5]. Dominant dirsekte fleksiyondan ziyade ekstansiyonda oluşan mekanik ağrı ve eklem hareket açıklığında azalma dirsek artrozu için karakteristiktir [6,7]. Ekstansiyonda kısıtlılıkla birlikte supinasyon ve pronasyonun etkilenmemesi de dirsek artrozunda tipiktir. Hastalığın geç dönemlerinde ise dirsek hareket arkının ortasında gelişen ağrı belirgindir.
