Simple SummaryCanine and human co-evolution have disclosed remarkable morphological plasticity in dogs. Brachycephalic dog breeds are increasing in popularity, despite them suffering from well-documented conformation-related health problems. This has implications for the veterinary caseloads of the future. Whether the recent selection of dogs with progressively shorter and wider skulls has reached physiological limits is controversial. The health problems and short life expectancies of dogs with extremely short skulls suggests that we may have even exceeded these limits. Veterinarians have a professional and moral obligation to prevent and minimise the negative health and welfare impacts of extreme morphology and inherited disorders, and they must address brachycephalic obstructive airway syndrome (BOAS) not only at the level of the patient, but also as a systemic welfare problem.AbstractThis article, written by veterinarians whose caseloads include brachycephalic dogs, argues that there is now widespread evidence documenting a link between extreme brachycephalic phenotypes and chronic disease, which compromises canine welfare. This paper is divided into nine sections exploring the breadth of the impact of brachycephaly on the incidence of disease, as indicated by pet insurance claims data from an Australian pet insurance provider, the stabilization of respiratory distress associated with brachycephalic obstructive airway syndrome (BOAS), challenges associated with sedation and the anaesthesia of patients with BOAS; effects of brachycephaly on the brain and associated neurological conditions, dermatological conditions associated with brachycephalic breeds, and other conditions, including ophthalmic and orthopedic conditions, and behavioural consequences of brachycephaly. In the light of this information, we discuss the ethical challenges that are associated with brachycephalic breeds, and the role of the veterinarian. In summary, dogs with BOAS do not enjoy freedom from discomfort, nor freedom from pain, injury, and disease, and they do not enjoy the freedom to express normal behaviour. According to both deontological and utilitarian ethical frameworks, the breeding of dogs with BOAS cannot be justified, and further, cannot be recommended, and indeed, should be discouraged by veterinarians.

Artificial selective breeding of man-induced breeds of dogs and cats has had major impacts mainly on the welfare of these animals. Although it spawned a wide variety of breeds, the modifications made in their anatomical structures inevitably entailed physiological and behavioral changes in some canine and feline breeds, bringing great consequences to them. By interfering in the anatomical conformation of the skull bones of dogs and cats by modifying the shape and size of the head, there is a narrowing of the airways, making it difficult to pass air. Consequently, respiratory disorders affected the animals with short snout causing the Brachycephalic Airway Obstruction Syndrome (Brachycephalic Obstructive Airway Syndrome-BOAS) or Brachycephalic Syndrome. This syndrome is characterized mainly by disorders of the airways, which lead to the obstruction of the same of progressive form. The abnormalities that compose them are composed of anatomical changes classified as primary and secondary that can be treated if identified, although not all of them can be corrected surgically. It is a progressive disease, leading to secondary changes that may be irreversible to the animal. Such changes are consequences of a process of chronic obstruction caused by the primary abnormalities of the syndrome. Due to the importance of its knowledge by the veterinarian, this work aimed to perform a literature review aimed mainly at the main anatomical abnormalities of the airways that contribute to the Brachycephalic Airway Obstructive Syndrome, and their respective surgical indications.

Nebulization of epinephrine to reduce the severity of brachycephalic obstructive airway syndrome in dogs

Objective: To determine the preoperative and postoperative effect of nebulized epinephrine on brachycephalic obstructive airway syndrome (BOAS) severity in dogs. Study design: Prospective clinical study. Sample population: Thirty-one client-owned pugs, French bulldogs, and English bulldogs with moderate to severe BOAS. Methods: Whole body barometric plethysmography was used to determine BOAS severity (BOAS index; 0%-100%) prior to and after nebulization with 0.05 mg/kg epinephrine diluted in 0.9% saline preoperatively. The same protocol was repeated postoperatively (within 24 hours of surgery). Results: Five dogs were excluded because they did not tolerate nebulization, and postoperative data were available for 13 dogs. Epinephrine nebulization resulted in a decreased BOAS index across all breeds of dog both before (9.6% [3.1% to −30.2%], n = 26) and after surgery (14.3% [0.9% to −24.3%], n = 13). The preoperative reduction in BOAS index was greater (17.3% [1.8% to −27.4%]) in dogs with a baseline BOAS index >70% (P = .006) and in pugs (16.9% [0.8% to −27.4%]) compared with French bulldogs (5.2% [3.1% to −30.2%], P = .03). Simple linear regression was used to identify a positive relationship between baseline BOAS index and reduction in BOAS index for pugs (n = 10, P = .001). Nausea was noted as a side effect in four dogs. Conclusion: Nebulized epinephrine reduced the BOAS index of dogs in this study. This effect was clinically significant in preoperative dogs with a BOAS index >70% and in dogs recovering from surgery. Clinical significance: This study provides evidence to support the use of nebulized epinephrine in the perioperative management of BOAS-affected dogs.
