Background: Percutaneous endoscopic gastrostomy (PEG) became the gold standard for enteral feeding. Currently, surgical gastrostomy is seldom used.Objective: Evaluating the role of surgical gastrostomy in a center with a large experience in PEG.Methods: A retrospective study ranged 13 years, collecting from clinical records: age, gender, underlying disease, date of procedure, technique, primary surgery, complications, 30-day mortality. Patients were divided according to indication for gastrostomy: a) neurological; b) head and neck cancer; c) other diseases; and d) drainage. PEG, open surgical and laparoscopic gastrostomies were compared concerning evolution of the number of procedures, characteristics of patients, complications and mortality.Results: We identified 509 PEG, 26 open and 8 laparoscopic surgical gastrostomies. An increasing number of the percutaneous approach over the years was observed, while the number of surgical gastrostomies remains steady (mean: 2.6/year). All percutaneous endoscopic gastrostomies but three were feeding procedures, mostly in neurological patients. All laparoscopic gastrostomies were feeding procedures in head and neck cancer. Most open surgical gastrostomies were secondary procedures, part of more complex surgeries, and frequently for drainage purposes. The open surgical approach displayed more morbidity and mortality, reflecting the severity of underlying diseases.Conclusions: In our institution, open surgical gastrostomy is seldom used, and mostly as part of complex procedures, frequently for drainage purposes. PEG is the choice to most dysphagic patients needing an enteral feeding access. When not feasible, laparoscopic gastrostomy is a suitable alternative.

ÖZETAmaç: Kritik hastaların bakım ve tedavisi sırasında enteral beslenme, enerji ihtiyacının karşılanması, vücut direncinin korunması, gastrointestinal sistem (GİS) florasının korunması ve translokasyonların önlenmesi açısından çok önemlidir. Bu çalışmadaki amacımız; hastanemizde PEG takılması planlanan hastaların PEG takılma endikasyonlarını, işlem sonrası komplikasyonlarını ve takipleri ile ilgili verileri sunmaktır.Yöntemler: Çalışmamızda Abant İzzet Baysal Üniversitesi Tıp Fakültesi Genel Cerrahi Anabilim Dalı endoskopi ünitesinde Ocak 2010-Aralık 2012 tarihleri arasında PEG takılması endikasyonu koyduğumuz 81 hasta retrospektif olarak değerlendirildi.Bulgular: Başarıyla PEG uygulanan hastaların 42'si (%56) erkek, 33'ü (%44) kadın idi. Hastaların yaş ortalaması 62+/-22.47idi. PEG uygulaması yapılan hastaların primer hastalıkları 45 (%60) hastada SVO, 4 (%5,3) hastada larinks ca, 2 (%2,6) hasta özofagus ca, 9 (%12) hasta epilepsi sonrası hipoksik iskemik ensefalopati, 3 (%4) hasta travma sonrası ensefalopati, 10 (%13,3) hasta solunum yetmezliği, 1 (%1,3) hasta nörobehçet, 1 (%1,3) hasta lenfosarkom tanılarına sahipti. Ortalama takip süresi 263+/-254,4 gün idi. Dört (%5,3) hastamız PEG işleminden sonraki ilk 1 ay içinde, 8 (%10,6) hastamız PEG işleminden sonraki 3 ay içinde primer hastalığı nedeniyle kaybedildi. Hastalarımızın hiçbirisinde primer olarak PEG işlemine bağlı ölüm görülmedi.Sonuç: Doğru endikasyon konulmuş olan hastalarda PEG uygulaması genel anestezi gerektirmemesi, yatak başında ya da endoskopi odasında uygulanabilir olması, kısa sürede uygulanması, mortalite oranın düşük olması, majör ve minör komplikasyonlar açısından daha avantajlı olması nedeniyle güvenli ve etkin bir şekilde kullanılabilir. ÖzetPerkütan endoskopik gastrostomi(PEG) ağız yoluyla beslenemeyen hastalara uzun dönem enteral beslenme sağlamak amacıyla karın duvarından mideye tüp yerleştirilmesidir. Bu çalışmamızda Genel cerrahi kliniğimizde PEG koyduğumuz hastaların PEG endikasyonlarını, erken ve geç dönemde karşılaştığımız komplikasyonları sunmayı amaçladık. 25 hastaya PEG yerleştirildi. En sık PEG konma nedeni 15 hasta ile (% 60) serebrovasküler hastalıklardı. 18 hasta evine tüp ile taburcu edilirken, 7 hasta hastanede izlemleri sırasında PEG işlemine bağlı olmayan sebeplerden kaybedildi. İşleme bağlı majör bir komplikasyonla karşılaşılmadı, erken dönemde görülen en sık görülen minör komplikasyon periostomal enfeksiyon iken geç dönemde ise tüp tıkanmasıydı. AbstractPercutaneous endoscopic gastrostomy (PEG) is to indwell a tube through abdominal wall into the stomach of patients who can not swallow orally for enteral feeding long term. In this paper we present indications and early or late term complications of PEG we encounter in our surgery unit. 25 patients underwent PEG procedure. The most common indication of PEG was cerebrovascular disorder in 15 (60%) patients.Of them, 18 were discharged, 7 were died of different causes during hospital stay. No major comlication was seen. Peristomal infection was the most common complication in early period but the tube occlusion in the late period. Abstract: The findings show PEG to be an efficient method for gastrostomy tube placement with a lower complication rate than SG. In addition, PEG is faster to perform and requires fewer medical resources. The authors consider PEG to be the primary procedure for gastrostomy tube placement.
