Compliance and adherence in glaucoma management

Abstract: Glaucoma is a preventable cause of blindness if timely effective and successful treatment is provided. Patient adherence to the medication is a constant challenge that is now recognized as an essential component to treatment. Several studies have demonstrated that patients are more likely to be adherent to their medication if they understand the disease and the rationale for treatment and if their treatment regimen is simplified. Additionally, using eye drops has its own set of challenges that must be recogniz… Show more

“…In India, it is the leading cause of treatable non-reversible blindness. [1] Primary open angle glaucoma (POAG) is defined as a chronic optic neuropathy with characteristic changes in the optic disc and visual field. Risk factors for POAG include older age, black race, family history (first-degree relative), thinner central corneal thickness, myopia, and elevated intraocular pressure (IOP).…”

“…In both cases, since a key function of aqueous humor is to bathe and nourish the avascular lens, cornea, and TM, reduced aqueous humor circulation through the conventional TM outflow pathway will further exacerbate the condition of the diseased tissue [1,3,7]. In addition, therapeutic compliance remains a very serious issue with published estimates of 27.8%, 50%, or even 75% noncompliance [45][46][47][48]. This is primarily due to side effects and the requirement for daily self-application of eye drops.…”

“…Many studies3137 have evaluated compliance with glaucoma medications, which consistently report, much like in other therapeutic areas, that compliance decreases as regimens become more complicated. Patients may decide that their adjunctive therapies are not important enough to use regularly and physicians may not realize that patients are not using them as prescribed.…”
