Complex Trauma in Children and Adolescents: Evidence‐Based Practice in Clinical Settings

Abstract: Complex trauma (CT) results from exposure to severe stressors that occur within the caregiver system or with another presumably responsible adult, are repetitive, and begin in childhood or adolescence. As a result, many of these children and adolescents experience lifelong difficulties related to self-regulation, relationships, psychological symptoms, alterations in attention and consciousness, self-injury, identity, and cognitive distortions. “…Developmental trauma, as described by van der Kolk, is complex trauma that occurs from significant and severe chronic traumatic events, experienced in childhood and adolescence, that have been perpetrated by a caregiver or individual that is expected to be a source of “security, protection, and stability” (Lawson & Quinn, ). The stress can be emotional, physical, sexual, or secondary (such as witnessing family violence), and can also include medical trauma and traumatic loss or grief.…”

“…Complex trauma theory (Cook et al, 2005;Courtois, 2004;Ford & Courtois, 2009;van der Kolk, 2005) posits that chronic adverse or stressful life events can create a lasting impact on the individual's ability to function physically or emotionally. When children experience repeated traumatic events, their developmental trajectory is significantly altered on several domains, including attachment, biology, affect regulation, dissociation, behavioral control, cognition, and self-concept (Cook et al, 2005). Rehabilitating the negative and often self-defeating behaviors that traumatized, delinquent youth exhibit may require a trauma-informed sensitivity to the difficulties experienced on these domains.…”

“…Also, large N group designs may obscure finer-grain elements of treatment unique to each case that may be critical to successful treatment. Some suggest the need to examine treatment for single cases based on evidence-informed interventions as a means of identifying important treatment dynamics, examining theoretical constructs, and to inform large n studies (Lawson and Quinn 2013). The case study below is an effort to contribute to this end.…”
