The 9p21.3 chromosomal region is a marker of the risk of cardiovascular diseases. The aim of this study was to analyze single-nucleotide polymorphism rs1333049 (chr9:22125504) in the population of Western Siberia (Russia) and possible associations with clinical and biochemical parameters. The population included in the analyses was selected from a sample surveyed within the framework of the Health, Alcohol and Psychosocial Factors In Eastern Europe (HAPIEE) study (9360 participants, >90% white, aged 45–69, males: 50%). In total, 2729 randomly selected patients were included. Plasma lipid levels were determined by standard enzymatic assays. Rs1333049 was analyzed by RT-PCR (BioLabMix, Russia). Frequencies of rs1333049 genotypes C/C (homozygote), C/G (heterozygote), and G/G were 0.22, 0.51, and 0.27 in this population. The Allele G frequency was 0.53. We found an association of allele G with total cholesterol and low-density lipoprotein cholesterol levels among male participants (p = 0.004 and p = 0.002, respectively). Allele C was significantly associated with the risk of myocardial infarction among the male participants (odds ratio 1.96, 95% confidence interval 1.14–3.38, p = 0.017) and the study population (odds ratio 1.83, 95% confidence interval 1.23–2.72, p = 0.004). Thus, rs1333049 is associated with myocardial infarction in the white population of Western Siberia (Russia).

Aim. To identify the factors of long-term adverse prognosis of patients with acute coronary syndrome as a result of five-year follow-up. Material and methods. The study included 280 patients with ACS hospitalized in the 1st cardiology Department of the Novosibirsk municipal CLINICAL hospital № 1 in 2010-2011. The study cohort included 145 patients with ACS (107 men and 38 women), 135 patients with ACS (93 men and 43 women). The average age of men was 56,3±5,2, women 52, 1±5,3 years. The criteria of the European society of cardiology (2015, 2017) were used for the diagnosis of ACS. For five years, all patients included in the study were contacted through communication and annual medical examinations, to which patients were invited to the clinic. The examinations included the following clinical and instrumental examinations: clinical examination, electrocardiography, Holter monitoring of electrocardiogram, echocardiography, lipid profile, inflammatory cytokines and molecular genetic parameters. In the study, a mathematical model for predicting five-year outcomes Oxpt and Oxbt. Results. Five-year observation allowed using the constructed mathematical model to determine not only the place of each factor in the cardiovascular prognosis, but also to assess how the role of these prognostic markers changes over time. Conclusion. The use of a mathematical model for predicting long-term adverse outcomes of ACS allows to assess the value of specific risk factors and predictors,

Цель. Установить ассоциации полиморфизмов генов-кандидатов, кодирующих компоненты эндотелиальной (NOS3, MTHFR) и симпатоадреналовой систем (ADRB1, ADRА2B) с ишемической болезнью сердца (ИБС) в когорте малочисленной популяции Горной Шории. Материал и методы. Проведено клинико-эпидемиологическое исследование в районах Горной Шории (п. Ортон, п. Усть-Кабырза, п. Шерегеш, г. Таштагол). Анализ факторов риска включал: учет возраста, пола, показателя индекса массы тела, окружности талии, наличие/отсутствие артериальной гипертензии (АГ), курения, наличия нарушений липидного и углеводного обменов. Диагноз ИБС выставлялся на основании трех эпидемиологических критериев: кодирования электрокардиограммы по Миннесотскому коду, опросника Rose и инфаркта миокарда в анамнезе. Обследуемым проведено молекулярногенетическое тестирование. Выделение ДНК из крови проводилась методом фенол-хлороформной экстракции. Статистическая обработка проводилась с помощью программ "STATISTICA 6.1" (StatSoft Inc., США) и SNPStats. Результаты. Распространенность ИБС в когорте шорцев составила 9,7%. Результаты нашего исследования установили взаимосвязь генетических маркеров симпатоадреналовой и эндотелиальной систем с коронарным атеросклерозом в коренной малочисленной популяции шорцев. С высоким риском развития атеросклеротической болезни сердца ассоциировался генотип 4b/4a гена NOS3 (ОШ 2,57; 95% ДИ (1,12-5,86), р=0,026) по доминантному типу наследования. Установлена взаимосвязь генотипа Т/Т гена MTHFR c ИБС по рецессивному типу наследования (ОШ 13,28; 95% ДИ (1,79-98,40), р=0,024). Заключение. Благодаря генетическим исследованиям, появляется реальная возможность не только проводить точную молекулярную диагностику, но и определять предрасположенность человека к тому или иному заболеванию. Выявление генетической предрасположенности к ИБС, может быть проведено задолго до появления клинических симптомов, что позволяет эффективно предупреждать её развитие или отодвигать сроки манифестации.
