Complex endovascular repair of postdissection arch and thoracoabdominal aneurysms

Abstract: Complex endovascular repair of postdissection aneurysms is a safe procedure in patients deemed unfit for open surgery. Our experience suggests that close follow-up is mandatory as secondary procedures are frequently required to completely exclude the false lumen.

“…Covering the thoracic, abdominal and infra-renal aorta with stent-grafts (during BEVAR, FEVAR) is sometimes too extensive and could lead to paraplegia due to spinal cord ischemia (Scali et al, 2013;Spear et al, 2018). Therefore, staging and other less invasive options are advised (Geisbüsch et al, 2014).…”

“…Although COOK dissection devices are available in size 46mm, an aorta which size is between 40-46mm is definitely too degenerated to believe that STABILISE could work. Those patients remain candidates for more risky procedures (BEFAR, FE-VAR) (Spear et al, 2018). Any previous endovascular interventions limit the effectiveness of the CERN concept.…”

“…Thirdly, reintervention, while it can be performed with very good outcomes in experienced aortic centres (11), still carries significant complications compared to equivalent first time aortic surgery in those institutions, let alone in the hands of the average cardiac surgeon. Even totally endovascular re-interventions carry significant risks of not only mortality, but stroke and paraplegia and are plagued by endoleaks (12). Finally, and most disturbingly, many of the aortic ruptures occurred in patients under close and regular surveillance (7).…”
