Objetivo: Difundir, por meio de uma revisão narrativa, conhecimentos sobre a síndrome do músculo piriforme, com ênfase em sua etiopatogenia, diagnóstico e tratamento. Métodos: No período de outubro de 2023, foi realizada uma busca nas seguintes bases eletrônicas de dados: PubMed, Scientific Electronic Library Online e ScienceDirect, utilizando os descritores padronizados pelo Medical Subject Headings e Descritores em Ciências da Saúde: “Síndrome do Músculo Piriforme”, “Síndrome del Músculo Piriforme” e “Piriformis Muscle Syndrome”, de artigos dos últimos 5 anos disponíveis na íntegra, totalizando 154 trabalhos. Após as etapas de inclusão/exclusão, foram selecionados 64 artigos para compor a revisão. Resultados: A síndrome do músculo piriforme é um subtipo da síndrome da dor glútea profunda que consiste em uma ciatalgia secundária a uma condição anormal do músculo piriforme. Apesar de acometer 2,4 milhões de novas pessoas por ano, ainda é uma condição subdiagnosticada. Possui uma patogênese diversa, com uma clínica composta principalmente por lombalgia e/ou dor na região glútea, que pode irradiar ao longo do trajeto do nervo isquiático. Além da anamnese e exame físico detalhados, o diagnóstico pode ser auxiliado por meio de exames de imagem, principalmente a ultrassonografia. O tratamento pode ser realizado com diversas modalidades, que incluem abordagens conservadoras, farmacológicas ou não farmacológicas, e cirúrgicas, tanto abertas quanto minimamente invasivas. Conclusão: É crucial que a síndrome do músculo piriforme componha a lista de diagnósticos diferenciais em quadros de ciatalgia e dor glútea, a fim de possibilitar um diagnóstico precoce e um manejo terapêutico adequado.

The lack of unified diagnostic criteria for piriformis syndrome until 2018 made it difficult to diagnose; therefore, it is often underestimated, and some patients do not receive appropriate treatment. Treatment methods include local anesthetic injection, steroid injection, and local botulinum toxin injection. However, the most effective drug for the treatment of severe piriformis syndrome has not been established. This report aimed to propose a new and more effective treatment for piriformis syndrome with a minimal risk of adverse effects. Case Reports: We performed ultrasound-guided hydro-dissection of the superficial and deep surfaces of the piriformis muscle under local anesthesia (a mixture of 18 mL of saline and 2 mL of 1% lidocaine) in 2 flexion, adduction, and internal rotation test-positive patients with tenderness of the piriformis muscle. In both patients, symptoms improved quickly after injection. One required hospital treatment but gradually returned to previous activities of daily living (ADL) status 5 days after admission and was then discharged. The other patient received 2 injections weekly to improve ADL status with continued lifestyle guidance. Conclusions: Hydro-dissection by ultrasound-guided injection of a very low concentration of local anesthetic is effective and has lower risk of adverse effects, thus making it more convenient for the treatment of piriformis syndrome than conventional treatments, such as local anesthetics, steroids, and botulinum toxin injection.
