“…This could explain the results of Ghysen et al [9] where a rise in TNF was seen during cuprophane HD but not during polyacrylonitrile (PAN) HD, but is not con sistent with the data from Herbelin et al [3] where plasma TNF levels, though elevated, were no different in patients exposed to cu prophane or PAN and did not change in either group before and after a dialysis. There was also no influence of type of mem brane on the increased intramonocytic IL-1 activity reported by Haeffner-Cavaillon et al [19]. Though extracellular IL-1 was low in the HD patients regardless of the dialyser membrane used reported by Blumenstein et al [15], cell-associated IL-1 was increased in patients treated with cuprophane and PAN sheet membranes though the mechanism of activation may differ with the two mem branes.…”

“…Direct cell-membrane interaction in the dialyzer has been favoured as the pri mary mechanism [5,22]. In addition, bacterial pyrogen in the dialysate [23] and activated complement components [24,25] are also thought to be involved in the cell stimulat ing process. Hollow fibre dialyzers, particulary those containing ethylene oxide sterilized regenerated cellu lose, have been shown to contain water-extractable sub stances which may also serve as a stimulus for monocyte activation [26,27].…”

“…It in duces polymorphonuclear leukocytes to re lease oxidative products and/or lysosomal enzymes [10]. C5a also promotes monocytes to produce interleukin-1 [16]. The leukocytedirected activities of C5a are retained even when its anaphylactic property has been abrogated by carboxypeptidase N.…”
