Background: Vascular targeted photodynamic therapy (V-PDT) is a safe and effective therapeutic modality for port-wine stains (PWS) by targetedly damaging the dilated and malformed blood vessels. This study aims to monitor and quantify the changes in oxygen saturation (StO 2 ), blood volume fraction (BVF) and perfusion in PWS lesions before and during V-PDT. Methods: Microvascular parameters (i.e., StO 2 and BVF) and skin perfusion were measured noninvasively by using diffuse reflectance spectroscopy (DRS) and laser Doppler imaging (LDI), respectively. The change in StO 2 , BVF and perfusion that occurred in the PWS lesions of 26 patients were monitored and investigated before and during V-PDT in vivo with the systematic administration of the porphyrin-based photosensitizer HiPorfin.Results: The mean StO 2 (P<0.05), BVF (P<0.05), and perfusion (P<0.001) in PWS lesions of all subjects significantly increased by 6%, 34%, and 113%, respectively, 3 min after the initiation of V-PDT. The StO 2 increased first and fluctuated during V-PDT. The overall trend of BVF change was consistent with the perfusion change. The BVF and the perfusion of PWS lesions increased after the initiation of V-PDT, and then gradually decreased.Conclusions: V-PDT is an effective therapeutic modality in treating PWS. Results showed that LDI and DRS permitted the noninvasive monitoring of the changes in StO 2 , BVF, and perfusion in PWS lesions during V-PDT, and these methods can be useful in facilitating our understanding of the basic physiological mechanisms during V-PDT.

Introduction: Photodynamic therapy is a minimally invasive clinical treatment modality for a variety of premalignant and malignant conditions combining a photosensitizing drug, oxygen and light irradiation. Hematoporphyrin is an organic photosensitizer, which mediates inhibition of endothelial cell proliferation and induces apoptosis. Human serum albumin is an endogenous drug carrier for hematoporphyrin. The present study aimed to investigate the hematoporphyrin binding to human serum albumin, which is its transport protein. Material and methods: The chemical reagents were hematoporphyrin (Hp), human serum albumin (HSA) and bovine serum albumin (BSA). In the experiment two techniques were used: spectrofluorimetry and UV-Vis absorption spectrophotometry. Results: The binding sites for Hp were identified in the tertiary structure of HSA by fluorescence quenching technique. The experiment with BSA delivered additional data on Hp-albumin interactions close to Trp135. The participation of tyrosyl residues apart from tryptophanyl ones was discussed. A decrease of the polarity in the binding sites, testifying to possible hydrogen bonding in the binding sites, was also described. The binding and quenching constants Hp-HSA and Hp-BSA were determined as well as the number of binding sites. Conclusions: Hp locates in subdomain IIA in the tertiary structure of HSA. The location in subdomain I close to Trp135 is also possible. Hp is also able to interact within tyrosyl residues.

A port‐wine stain (PWS) is a congenital capillary malformation that is seen in 0.3–0.5% of newborns. Although many types of lasers have been used to treat PWSs, few studies have investigated the efficacy of 577‐nm Pro‐Yellow lasers. The aim of this study was to establish the efficacy of Pro‐Yellow lasers in PWS treatment. In total, 26 patients, comprising 15 females (57.7%) and 11 males (42.3%) with a mean age of 24.7 ± 11.8 (range 1–50) years were included in the study. All were treated with a 577‐nm Pro‐Yellow laser at 4‐week intervals, 3–10 times. The sessions initially used scan mode and a fluence dose of 24 J/cm2 per session, on average. The mean dose was increased by 4 J/cm each session and the maximum dose administered was 44 J/cm2. A mean regression of 68.8 ± 13.9% in the size of the lesions over 5.23 ± 2.7 sessions was observed. There was no statistically significant difference between the female and male patients when they were compared in terms of the number of treatment sessions (p = .789) and treatment success (p = .39). These case series demonstrate that the Pro‐Yellow laser is a safe and well‐tolerated treatment for PWSs. However, deep‐seated lesions required more treatment sessions and it was observed that clinically the lesions did not completely disappear.
