Comparison of Thyroid Transcription Factor-1 and Hepatocyte Antigen Immunohistochemical Analysis in the Differential Diagnosis of Hepatocellular Carcinoma, Metastatic Adenocarcinoma, Renal Cell Carcinoma, and Adrenal Cortical Carcinoma

Abstract: We compared the effectiveness of thyroid transcription factor-1 (TTF-1, cytoplasmic reactivity) and hepatocyte antigen (HPA) as markers for characterization of hepatocellular carcinoma (HCC) and as discriminators to distinguish HCC from its histologic and cytologic mimics. Formalin-fixed, paraffin-embedded sections of 258 specimens, including 76 HCCs, 85 metastatic adenocarcinomas, 75 renal cell carcinomas (RCCs), and 22 adrenal cortical carcinomas (ACCs), were evaluated. Specimens included tissue sections and… Show more

“…16,24 Many authors reported on the immunoreactivity spectrum of HepPar1 in formalin-fixed tissue from a variety of neoplasms. Lugli et al 24 found positivity in 35 of 48 HCCs (73%), Chu et al 25 in 88 of 96 HCCs (92%), Lee et al 26 in 60 of 75 HCCs (80%), and Wieczorek et al 27 in 50 of 76 HCCs (66%). In our study, HepPar1 was positive in all HCCs, as Siddiqui et al 28 have also reported.…”

“…16 This antibody has shown some nonspecific cytoplasmic staining and suspected to crossreact with a cytoplasmic protein in liver cells. 17 In the current study, we also used another monoclonal antibody, the SPT24 clone, to detect the expression of TTF-1. In total, 10% of lung-metastatic colorectal adenocarcinoma and 5% of primary colorectal adenocarcinomas were found positive, respectively, these results dramatically differ from the literature, which showed no positive cases in a total of 131 primary colonic carcinomas and 52 metastatic carcinomas in the lung (Table 1).…”

“…Most adenocarcinomas are positive for carcinoembryonic antigen, epithelial membrane antigen and CD15 while adrenocortical carcinomas are negative [7,27]. Adrenocortical carcinomas are also negative with the renal cell carcinoma (RCC) and hepatocyte antibodies [19,31] (Table 1). …”
